氯沙坦对高血压患者24h动态血压的影响 被引量:2

Effects of Losartan on 24 Hour Ambulatory Blood Pressure in Patients with Essential Hypertension

摘要 采用动态血压监测 (ABPM)法比较 3 0例轻中度高血压病患者经氯沙坦 (5 0mg/d)治疗 4周后 2 4h平均血压、白昼及夜间平均血压、心率、血压负荷、夜间下降率等的变化 ,并与诊室血压 (CBP)相比较。治疗 4周末 ,CBP、2 4h平均血压、白昼及夜间平均血压、血压负荷均有显著降低 ,心率无显著改变。CBP示 4周末总有效率为 70 .0 %。各时点血压均较治疗前下降 ,对夜间血压的影响略小 ,但血压昼夜节律无显著改变。降压疗效谷 /峰比大于 70 %。提示 :氯沙坦能安全有效地降低血压 ,维持正常的血压昼夜节律 ,尤适于夜间血压低的杓型高血压患者。 Thirty patients with mild to moderate essential hypertension were treated with losartan (50 mg/d) for 4 weeks. Clinical blood pressure (CBP) and ambulatory blood pressure monitoring (ABPM)were performed at before and after the treatment to compare the mean blood pressure of 24 h, daytime and night, the load of the blood pressure and the circadian rhythm. Results: CBP, the load of blood pressure and the mean blood pressure of 24 h, daytime and night were significantly reduced. No significant difference was found in heart rate. The effective rate dependent CBP was 70.0%. There were significant decreases in every hour blood pressure. There was less effect on night blood pressure, but the circadian rhythm wasn′t significantly changed. The trough to peak ratios for SBP and DBP were more than 70%. The result shows that losartan could significantly and safely decrease the blood pressure. It might maintain normal circadian rhythm of the blood pressure.
