^(18)F-FDG PET, EEG and MRI examinations in the localization of epileptic foci before operation: a comparative study

摘要 目的 比较癫痫发作间期18F-FDG PET显像、EEG、MRI在手术前癫痫灶定位中的价值。方法 对24例难治性癫痫患者,术前行发作间期18F-FDG PET脑显像、EEG、MRI检查,术中行皮层脑电图(ECoG)检查,术后送组织病理检查,比较其结果。结果 24例术前发作间期18F-FDG PET脑显像23例检出癫痫灶,而EEG、MRI检出的阳性率分别为83.3%、16.7%,PET与ECoG阳性检出符合率为83.3%,EEG与ECoG阳性检出符合率为41.7%,PET和EEG均阳性与ECoG阳性符合率为100%。结论 发作间期18F-FDG PET脑显像是诊断和定位癫痫灶灵敏而有效的方法,对制定癫痫的手术方案具有重要意义,其对癫痫灶定位的价值优于EEG和MRI。 Objective To evaluate the diagnostic value of interparoxysmal examinations with 18F-FDG positron emission tomography (PET), electroencephalography (EEG) and magnetic resonance imaging (MRI) in the localization of epileptic foci before operation. Methods Preoperative 18F-FDG PET, EEG and MRI examinations were performed in 24 patients with refractory epilepsy, who also received electrocorticography (ECoG) during the operation. All the examination results including that of postoperative pathological examination of the excised tissues were compared to decide their diagnostic values. Results Interparoxysmal 18F-FDG PET imaging displayed the epileptic foci in 23 patients, and the rates of positive findings by EEG and MRI were 83.3% and 16.7% respectively. With PET examination, the localization of the epileptic foci was confirmed by ECoG at a rate up to 83.3%, while the rate was 41.7% with EEG. In cases that EEG and PET results were both positive, the rates reached 100%. Conclusions 18F-FDG PET imaging is sensitive and effective in epileptic foci detection and localization with much superiority over EEG and MRI, and it may assist in designing the surgical protocol for refractory epilepsy.
