摘要
目的 探讨甲基泼尼松龙对周围神经再生的促进作用及剂量选择。 方法 2 8只成年SD雌性大鼠 ,取股外侧切口 ,钳夹造成双侧坐骨神经损伤。创伤后 1h起分别肌注不同剂量的甲基泼尼松龙或对照溶液 ,并间隔 3h维持肌注 ,持续 2 4h。按肌注剂量的不同随机分为大剂量组 (首剂15 0mg/kg)、中剂量组 (首剂 30mg/kg)、小剂量组 (首剂 15mg/kg)和对照组 (生理盐水与 0 9%苯甲醛1:1混合液 )。肌注后观察各组大鼠一般状态 ,记录术前及术后 4周大鼠足印 ,测定坐骨神经功能指数 (SFI) ;术后 4周显露双侧坐骨神经测定神经传导速度 (NCV) ;对取得的数据进行统计学分析。同时行神经横切片组织学观察。 结果 中、小剂量组SFI均值优于大剂量组和对照组 ,但差异无显著性意义 ;中、小剂量组神经诱发电位引出率 (10 0 % )明显高于对照组 (5 7 1% ) (P <0 0 1) ;小剂量组NCV值优于其他三组 ,而且差异具有显著性意义 (P <0 0 5 ) ;大剂量组大鼠均出现明显神经精神症状 ,有 1只因感染死亡 ,3只 4侧出现局部感染 ,其他三组未见明显异常。中、小剂量组神经横切片轴索再生情况优于大剂量组及对照组。 结论 甲基泼尼松龙的全身应用可有效促进周围神经损伤后再生 ,但剂量选择不应过大 。
Objectives [WTo investigate the effects of methylprednisolone on repair of peripheral nerve injury and discuss the dose selection. [WT5”HZ]Methods[WT5”BZ] A total of 28 female SD rats were randomized into four groups. Their sciatic nerves on both sides were injured with the clamp. Three doses of methylprednisolone were applied to the rats in three experimental groups and a 1:1 normal saline and 0 9% benzyl alcohol mixture to those in the control group. The drug was intramuscularly injected into bilateral hind legs in turn. The first doses of 150 mg/kg, 30 mg/kg and 15 mg/kg methylprednisolone were given to the rats in the three experimental groups within one hour after the injury. In the following 24 hours, three doses of 75 mg/kg, 15 mg/kg and 7 5 mg/kg methylprednisolone were given to the rats in the three experimental groups every 3 hours. All the rats were observed for 4 weeks and electrophysiological test was conducted and sciatic nerve function index (SFI) determined 4 weeks after the surgery. [WT5”HZ]Results[WT5”BZ] SFI was higher in medium and low dosage methylprednisolone groups than in the other two groups. However, there was no significant difference ( P >0 05). Nerve conductive velocity (NCV) was significantly faster in low dosage group than in other three groups ( P <0 05). Nerve axonal regeneration was better in low and medium dosage groups than in high dosage and control groups. Four rats in high dosage group got local infection and one of them died on the 9th day after surgery. [WT5”HZ]Conclusions[WT5”BZ] Methylprednisolone can facilitate the repair of peripheral nerve injury in rats, but the dose should not be very large.
