摘要
目的 观察西立伐他汀对高胆固醇患者单核 内皮细胞黏附的影响 ,以探讨他汀类药物其他可能的抗动脉粥样硬化作用。方法 检测 17高胆固醇患者服用西立伐他汀前后的外周血单核细胞与培养人脐静脉内皮细胞的黏附率。结果 17例高胆固醇患者 ,男 15例 ,女 2例 ,年龄 36~ 5 4岁。口服西立伐他汀 0 .3mg/d ,8周后 ,高胆固醇患者的总胆固醇降低 19.6 % (P =0 .0 0 1) ,低密度脂蛋白胆固醇 (LDL C)降低 31.1% (P =0 .0 0 1) ,甘油三酯 (TG)降低 5 .1% (P =0 .0 0 97) ,单核 内皮细胞黏附率降低 2 7.7% (P =0 .0 0 1) ;高胆固醇患者的单核 内皮细胞黏附率与总胆固醇 (r=0 .6 744 ,P =0 .0 0 3)、LDL C(r=0 .6 175 ,P =0 .0 0 8)、TG(r=0 .712 7,P =0 .0 0 13)呈正相关性 ;内皮细胞先经西立伐他汀孵育后 ,对单核细胞的黏附率与高胆固醇患者服药前的黏附率差异无显著性 (P =0 .7470 ) ;单核细胞经西立伐他汀孵育后 ,与内皮细胞的黏附率较服药前降低 12 .9% (P =0 .0 0 1) ,但仍较服西立伐他汀 8周后高 2 0 .7% (P =0 .0 0 1)。结论 高胆固醇患者经西立伐他汀治疗血脂水平下降后 ,其单核 内皮细胞黏附率随之降低 。
Objective To observe the effect of cerivastatin on monocyte adhesion to endothelial cells in hypercholesterolemia patients and to explore the other antiatherosclerosic role of cerivastatin besides lowing plasma cholesterol level. Methods The rate of monocyte adhesion to endothelial cells (RMAEC) was assessed before and after the oral cerivastatin therapy (0.3 mg/d×8 weeks)in 17 patients (15 male and 2 female, with 36 54 years old )of hypercholesterolemia, in which the monocytes were taken from the perapheral blood of these patients and the endothelial cells came from cultured human umbilical cord vein.Results After the cerivastatin therapy, the total cholesterol (TC) level, low density lipoprotein(LDL) and triglyceride (TG) levels decreased by 19.6%( P =0.000?0), 31.1%( P =0.001) and 5.1%( P =0.009?7) respectively in those patients. The RMAEC was reduced by 27.7%[(21.43±2.47)% vs (29.66±4.38)%, P =0.000?1]. There was a positive correlation between RMAEC and the level of TC( r =0.674?4. P =0.003),LDL( r =0.617?5, P =0.00?8) or TG ( r =0.712?7, P =0.001?3)respectively. After preincubation of endothelial cells with cerivastatin, the RMAEC was found no significant difference compared with that before cerivastatin therapy in these patients, while after preincubation of monocyte with cerivastatin,the RMAEC was found 12.9% lower ( P =0.011?4)than that before the cerivastatin therapy,but still 20.7% higher than that after the 8-week therapy( P =0.000?1).Conclusion The RMAEC decreased following the plasma lipids lowering by cerivastatin in hypercholesterolemic patients. There may be a direct effect of cerivastatin on monocyte which could inhibit the RMAEC.
