摘要
目的：探讨机械通气（ＭＶ）在治疗重症慢性呼吸衰竭患者失败的原因，以及降低治疗失败率相应的措施。方法：通过对治疗失败组２０例与治疗成功组３０例的多种指标进行对照分析研究，分析治疗失败的主要原因。结果：治疗失败组中，在上机前重症感染、意识障碍、低蛋白血症、肾功能不全，以及上机后出现心血管并发症、气压伤、低氧合指数的发生率明显高于成功组（Ｐ＜０．０５）。结论：上机前重症感染、意识障碍、低蛋白血症、肾功能不全，以及上机后出现心血管并发症、气压伤、低氧合指数与治疗失败有关。
Objective: To investigate the causes of treatment failure in severe chronic respiratory failure pa- tients who were given mechanical ventilation. Methods :50 patients divided into two groups were enrolled in this study. Failure group consisted of 20 patients and success group,30 patients. The indexes measured in both groups were compared with cross * check analysis. Results :The incidence of severe infection distrubance of conscious- ness hypoalbuminemia and renal insufficiency before MV and cardiovascular complication, barotrauma and hy- pooxynation index during MV are significantly higher in failure group than in success group. Conclusions: se- vere infection disturbance of consciousness, hypoalbuminemia and renal insufficiency before MV and cardiovascu-lar complication barotrauma and hypooxygenation index during MV are main factors associated with the treat- ment failure.
