摘要
研究早期糖尿病视网膜病变 (diabeticretinopathy,DR)的视网膜电图震荡电位 (Oscillatory potentials,OPs)和图形视网膜电图 (Pattern electroretinogram,P- ERG)的变化特点 ,借以早期诊断 DR。方法 :对 2 0例 (40眼 )正常人和 5 7例 (113眼 )糖尿病病人进行 OPs和 P- ERG检测。结果 :当糖尿病病人眼底尚未出现改变时 ,P- ERG的 b波波幅和 OPs各子波波幅及总波幅已降低。随着 DR的发展 ,各参数的异常率逐渐上升 ,各波波幅也随之下降 ,潜伏期延长。OPs的 O3波波幅和总波幅以及 P- ERG的 b波波幅较其它指标具有较高的敏感性 ,其中 O3波波幅为最敏感的指标。结论 :OPs的 O3波波幅和总波幅等参数能够作为视觉电生理的功能性指标预测 DR的发生 ,在 DR的早期诊断和评价
Objective: To study the changes in oscillatory potentials (OPs) of electroretinogram (ERG) and pattern electroretinogram (P-ERG) in early diabetic retinopathy (DR), and so as to diagnose early DR in time. Methods: The OPs of ERG and P-ERG were measured in 20 cases (40 eyes)of normal subjects and 657 cases (l13 eyes) with diabetes mellitus (DM). Results: The results showed that the b-wave amplitude of P-ERG and O1, O2, O3 and O4 as well as ΣO amplitudes of OPs had significantly decreased in DM patients even though their ocular fundus had not changed. With the development of DR, the abnormal rates of the parameters increased gradually, the amplitudes decreased correspondent to the period of incubation extending. These parameters, such as O3 and ΣO amplitudes of OPs and b-wave amplitude of P-ERG, were more sensitive than the other parameters, and the O3 amplitude of OPs was the most sensitive among the parameters. Conclusions: As the functional parameter of visual electro-physiology, these parameters can be used to predict the progress of DR. It will have, therefore, clinical practical value for the diagnosis of early DR and the evaluation of the DM curative effect.
