摘要
目的 :研究联合使用异丙酚和异氟醚对大鼠心室肌细胞膜L 型钙离子通道的影响 ,阐明异丙酚和异氟醚在离子通道水平上对心肌细胞抑制的药理作用机制。方法 :采用急性酶解法获得SD大鼠的单个心室肌细胞 ,在保持膜片钳钳夹电位 - 4 0mV ,波宽为 2 50ms的条件下 ,利用全细胞膜片钳技术 ,观察不同浓度的异丙酚、异氟醚及其二药复合应用时对心肌细胞钙离子通道电流的影响。结果 :用含有1μmol/L四已铵的台氏液灌流时 ,可记录到具有电压和时间依赖性内向的钙电流 ;加入 1μmol/L维拉帕米时 ,该电流被迅速消除。 12 μmol/L、2 5μmol/L异丙酚和 7× 10 2 μmol/L、1 1× 10 3μmol/L异氟醚对钙电流无显著性影响 (P >0 0 5) ;50 μmol/L、2 50 μmol/L异丙酚和 2 2× 10 3μmol/L、4 4× 10 3μmol/L异氟醚对钙电流有显著性影响 (P <0 0 5) ;2 5μmol/L异丙酚和 1 1× 10 3μmol/L异氟醚复合剂、12 5μmol/L异丙酚和 2 2× 10 3μmol/L异氟醚复合剂对钙电流有显著性影响 (P <0 0 5)。 结论 :异丙酚、异氟醚及其两药复合应用时对心肌产生负性肌力作用有浓度依赖性 ,其机制与其抑制钙离子通道电流有关。
Objective To observe the effect of the propofol and isoflurane on L type calcium channel in isolated ventricular myocytes.Methods Single venticular cell of SD rat was obtained by enzymatic dissociation method.Under the condition of clamp-40 mV with duration of 250 ms,the whole cell patch clamp recording of technique was used to record the change of calcium current following the adiministrating of propofol,isoflurane and the combination of these two drugs.Results The verapamil sensitive L type calcium inward current was recorded in the venticular myocytes.12 μmol/L,25 μmol/L of propofol and 7×10 2 μmol/L,1 1×10 3 μmol/L of isoflurane did not change the peak ica( P >0 05),50 μmol/L,250 μmol/L of propofol and 2 2×10 3 μmol/L,4 4×10 3 μmol/L of isafurane,the combination of propofol 25 μmol/L and 1 1×10 3 μmol/L of isoflurane,the combination of propofol 125 μmol/L and 2 2×10 3 μmol/L of isoflurane decreased L type calcium peak current( P <0 05).Conclusion The negative motorial action of propofol,isoflurane and combination of two drugs on the heart is related to the inhibition of L type calcium current.
