摘要
目的 探讨高压氧（ＨＢＯ）辅助治疗小儿急性中、重度氟乙酰胺中毒的疗效。方法 ７１例患者随机分成两组：对照组３６例，采用常规治疗（洗胃、使用特效解毒剂乙酰胺、抗感染、止痉、能量合剂等）；ＨＢＯ组３５例，在上述治疗基础上辅以ＨＢＯ治疗，即将患儿置于加压舱内停留 １００ ｍｉｎ，治疗压力０．２ ＭＰａ（２ ＡＴＡ），家长助其戴面罩吸纯氧５０ ｍｉｎ，中间休息吸空气１０ ｍｉｎ，每日１次，１０次为 １个疗程，一般只需治疗５～１０次。结果 ＨＢＯ组昏迷清醒时间（ｈ）明显快于对照组，有非常显著性差异（５２．６６±１９．０２ｖｓ７１．０３±３４．０４，ｔ＝２．７９，Ｐ＜０．０１）；ＨＢＯ组治愈天数比对照组明显缩短，有显著性差异（９．１±２．ｌｖｓ１２．８±４．１，ｔ＝２．５７，Ｐ＜０．０５）；ＨＢＯ组治愈率８８．６％高于对照组６９．４％，有显著性差异（Ｘ～２＝３．８９ Ｐ＜０．０５）。结论 ＨＢＯ辅助治疗小儿急性氟乙酰胺中毒，对促使昏迷患儿尽快清醒、缩短病程和提高疗效作用显著，值得临床推广应用。
Objective To explore the effect of adjuvant hyperbaric oxygenation(HBO)treatment in children with acute moderate and severe fluoroacetamide poisoning. Methods 71 patients were ran- domly divided into two groups: 36 patients of control group were treated with routine therapy (gastric lavage, use of specific antidote acetamidum, anti-infection, anti-convulsion, energy-mixture, etc.); while 35 patients in HBO group were treated with HBO besides the routine therapy, practically, the patients were sent into HBO chamber and were stayed for 100 min. The pressure was 0.2 MPa(2 ATA) and pure oxygen was inhaled for 50 min with 10 min break by mask with the help of their parents. The HBO ther- apy was carried out once a day,and 10 times as a course. Generally, 5 to 10 times were needed. Results The recovery time in hour of the patients with coma in HBO group was significantly shorter than that in control group (52.66±19.02 vs 71.03±34.04, t= 2.79, P<0.01). The days of cure in HBO group were also significantly shorter than that in control group(9.1±2.1 vs 12.8±4.1, t = 2.57,P<0.05). And the cure rate of HBO group was significantly higher than that of control group(88.6% vs 69.4%, X-2= 3.89,p<0.05).Conclusions The effect of adjuvant HBO treatment in children with acute fluo- roacetamide poisoning is remarkable with the coma patients recovered quickly,and it shorten the disease course and increase the curative effect. It is recommended for wilder clinical application.
