摘要
目的探讨高压氧(HBO)对大鼠急性局灶性脑缺血再灌注(IR)损伤的影响。方法应用血管内细丝栓堵大脑中动脉(MCA)的局灶性脑缺血大鼠模型,利用组织病理学方法,观察了MCA缺血1小时,再灌注4,11,23,71小时脑梗塞区范围、神经元损伤程度及中性白细胞浸润的变化,并在常压纯氧治疗组和HBO组分别应用常压纯氧(99.5%)和0.25MPa(ATA)HBO于再灌注期间各分别治疗1,2,3,5次后比较观察各组以上各时间梗塞区范围、神经元损伤程度及中性白细胞浸润的变化。结果常压纯氧组和缺血再灌注(IR)组相比梗塞区范围、神经元损伤程度及中性白细胞浸润无明显差异;而HBO组和IR组相比梗塞灶面积和神经元损伤数目在再灌注71小时(治疗5次后)分别减少了8.43%(视交叉前平面)、11.20%(视交叉平面)、8.66%(视交叉后平面)和每5个高倍(×400)视野11.36个(视前区)、8.94个(纹状体内侧区)、14.25个(皮层区),两组间存在显著性差异(P<0.05)。视交叉平面的视前区、纹状体与皮层的中性白细胞在MCA缺血后5小时出现,分别为每5个高倍(×100)视野3.84、4.61与2.56个,在12小时轻度增?
Objective To investigate the effect of hyperbaric
oxygen(HBO) on focal cerebral ischemiareperfusion(IR) injury in rats. Methods Rat model of
focal cerebral ischemia induced by intraluminal filament occlusion of middle cerebral
artery(MCA)was used.Histopathological changes of infarct size,neuronal damage and neutrophil
infiltration in the ischemic brain area after MCA occlusion for 1 hour and reperfusion for 4,11,23
and 71 hours were examined,and during the reperfusion period,normal pressure pure
oxygen(99.5%O\-2)or 0.25 MPa(ATA)HBO treatment for 1,2,3 and 5 times were applied to
respective groups and their results compared. Results The infarct size,neuronal damage and
neutrophil infiltration in normal pressure pure O\-2 group were similar to those of
ischemiareperfusion(IR) group at 71 h.Whereas the infarct size and neuronal damage in HBO
group after 5 times of HBO treatment at 71 h for 8.43%(level before optic chiasma),11.20%(level
of optic chiasma)and 8.66%(level behind optic chiasma),and the numbers of neuronal damage in
5 high power(400)fields(Nr/5F)were 11.36(preoptic area),8.94(medial striatum),and
14.25(cortex)less as compared with IR group respectively.Besides,the neutrophil
numbers(number/5 fields,100)in preoptic,striatum and cortex at the level of optic chiasma were
384,461,256 at 5 h,664,734,572 at 12 h and 978,1048,784 at 24 h after the IR respectively;and
they were much lower in HBO group at 12 h(5.01,5.22,4.03)and 24 h(7.13,7.21,6.14)as compared
with IR group(P<005)respectively. Conclusions HBO(0.25 MPa,5 times)has significant effects of
reducing infarct size,lessening neurological symptom evaluation score,inhibiting neutrophil
infiltration and decreasing the neuronal damage inpenumbraarea on acute focal IR injury in
rats.
