摘要
目的 :比较判断不同灌注方法和心脏停搏液对缺血心肌保护作用的效果。方法 :采用三种不同灌注液和灌注方法对 2 1例离体家兔心脏在Langendorff装置中进行缺血心肌灌注实验 ,分别检测不同时间的心肌标本中心肌肌动蛋白 (actin)含量 ,观察缺血心肌细胞的超微结构改变。结果 :当心肌缺血 6 0min、90min ,actin阳性物含量在温血灌注组减少不明显 7 96± 1 2 3 ,6 84± 1 92 ,而在冷晶体灌注组减少明显 6 75± 1 15 ,6 0 1± 1 0 1,经统计学处理有显著差异 (P <0 0 1)。同样 ,缺血心肌细胞超微结构的改变也以冷晶体灌注组明显 ,可见心肌细胞内和细胞外间质明显水肿 ,线粒体肿胀 ,肌原纤维排列紊乱 ,有肌丝断裂 ,肌节明暗带模糊、部分消失 ,糖原含量明显减少。并随缺血时间延长 12 0min变化更显著。结论 :应用温血灌注法对缺血心肌的保护效果优于冷晶体灌注法。
Objective:To compare and judge the protective effections of isolated rabbit's ischemic heart with the different perfusing methods and cardioplegias. Methods:Immunohistochemical alteration of cytoskeletal actin were measured and ultrastructural changes were observed in 21cases of ischmic rabbit's heart used by the methods of perfusion with warm blood cardioplegia and cold crystalloid cardioplegia. Results:The actin positive contents was 7 96±1 23 and 6 84±1 92 in warm blood groups when the ischemic time was 60 min and 90 min, while 6 75±1 15 and 6 01±1 01 in the cold crystalloid group. The difference is statistically significant (P<0 01). The obvious changes of the ultrastructure in the cold crystalloid group was also observed . The most typical features in this group are increased intra-and extracelluar edema,mitochondrial swelling, disruption of microtubules. Those changes are more obvious as time goes on. Conclusion:The research indicated that the warm blood cardioplegia would provide better myocardial preservation than cold crystalloid cardioplegia.
