摘要
为了探讨颈动脉体瘤的诊断及手术的可靠性、安全性 ,对 14例颈动脉体瘤的资料进行回顾性分析 .14例中 5例行普通颈动脉造影、 9例行DSA检查 ,均确诊为颈动脉体瘤 .7例未行手术 ,7例分别在行Matas压迫训练 30～ 40d前后 ,DSA和TCD检查显示已建立良好的侧枝循环 .4例手术剥离摘除肿瘤 ;1例肿瘤及受累颈内动脉切除并结扎颈内动脉 ;1例肿瘤加颈内动脉切除、颈内动脉端端吻合 ;1例肿瘤加颈总动脉、颈内、外动脉部分切除 ,颈内静脉搭桥颈内动脉—颈总动脉吻合 .结果 :7例患者手术成功 ,无偏瘫、失语等并发症 .结果表明 :Matas训练是促使大脑侧枝循环建立的有效而可靠的试验 ;DSA不仅诊断颈动脉体瘤可靠而且可直观了解颈总动脉压迫训练后颅内侧枝循环的建立情况 ;TCD可以了解侧枝循环的建立及脑内供血情况 ,指导手术治疗 .上述三项的结合是颈动脉体瘤手术成功的必备条件 .
To study the diagnosis and reliability safety of the surgery of carotid body tumors. 14 cases were analyzed. 5 of them were proved by ordinary angiography and the other 9 cases were identified by digital subtracted angiography (DSA). 7 of the l4 cases refused operation, the other 7 cases had utilized Matas training for 30～40 days before the operation. Then DSA and transcranial Doppler ultrasound (TCD) showed that the good collateral circulation were built in the 7 patients. The tumors were stripped and remove in 4 cases and were removed involved internal carotid artery in 3 cases. 1 of the 3 cases was ligated internal carotid artery, 1 of which was made anastomosis of internal carotid artery, the other 1 was used internal jugular vein grafting common-infernal carotid artery anastormosis. 7 cases all succeeded in operation. They had no hemiparalysis, aphasia and other complications. DSA is not only reliable and perfect method on diagnosis of carotid body tumor, but also it can directly observe the building of collateral circulation. Matas training can efficiently promote the collateral circulation. TCD can verify the collateral circulation. Combining the three methods is reliable and safe basis for successful surgery.
