p53和ras基因突变及HPV感染与喉鳞癌的关系

摘要 ①目的 探讨人乳头瘤病毒 16 ,18型 (HPV16 ,18)感染 ,抑癌基因p5 3和癌基因ras突变与喉鳞癌的关系 ,并初步分析 3种致癌因素的相互关系。②方法 应用聚合酶链反应 (PCR)和限制性片段长度多态性 (RFLP)分析技术 ,检测 40例喉鳞状细胞癌组织中 p5 3基因第 5 ,7外显子及ras基因 (N ras ,K ras,H ras)第 12密码子的突变情况 ,同时用PCR技术检测 40例标本中HPV16 ,18的感染情况。③结果  40例标本检出HPV16阳性标本 14例 ,HPV18阳性标本 4例。 4例标本 p5 3基因第 5外显子第 143密码子发生突变 ,其中 1例HPV16阳性 ,p5 3基因第 7外显子未发现突变。 9例标本ras基因发生突变 (N ras 6例 ,K ras 2例 ,H ras 1例 ) ,其中 2例HPV16阳性 ,但未见 p5 3与ras基因同时发生突变者。HPV阳性和阴性标本中p5 3基因突变率、ras基因突变率的差异均无显著性(χ2 =0 .10 1,1.392 ,P >0 .0 5 )。④结论 HPV感染、p5 3和ras基因突变与喉鳞癌的发生密切相关 ,三者之间的关系尚待进一步探讨。 Objective\ To study the role of human papillomavirus type 16,18(HPV16,18) infection, p53 gene mutation and ras gene mutation in laryngeal squamous cell carcinoma and to investigate the association of p53 gene mutation, ras gene mutation and HPV. \ Methods\ The polymerase chain reaction(PCR) combined restriction fragment length polymorphism analysis(PCR RFLP) technique was used to analyze DNA fragments containing exon 5 and 7 of p53 and codon 12 of N , K , and H ras in 40 laryngeal squamous cell carcinoma tissues.HPV16 and 18 DNA were also amplified by PCR. \ Results\ HPV16 and 18 DNA were found in 14 cases and in 4 cases respectively. 4 of 40 cases had a point mutation in exon 5 of p53 gene.No mutation happened in exon 7 of p53 gene. 9 of 40 cases revealed ras mutations(6 in N ras, 2 in K ras and 1 in H ras). 1 case with p53 gene mutation and 2 cases with ras gene mutation were also HPV positive. No significant differences were found in both of the rates of p53 gene mutation and ras gene mutation between HPV positive cases and HPV negative cases( χ 2=0.101,1.392,P >0.05). \ Conclusion p53 gene mutation, ras gene mutation and HPV16,18 infection were closely associated with the development of laryngeal squamous cell carcinoma, but the correlation of the three should be further investigated. [
