摘要
目的 :了解温阳健心灵口服液 (WYJXL D)对收缩功能不全性心力衰竭 (心衰 )的疗效。方法 :12 0例患者随机分 2组 ,实验组给予 WYJXL D、对照组给予开搏通治疗 ,疗程均为 2周。治疗前后均做多普勒超声心功能检查 ,部分患者做心钠素 (ANF)、内皮素 (ET)和甲状腺激素 (FT3、FT4)测定。治疗后按卫生部颁布的《中药新药治疗充血性心衰指导原则》进行疗效判定。结果 :实验组总有效率 6 7.8% ,对照组 6 0 .0 % ,2组疗效经 Ridit分析无显著差异。实验组治疗后心脏收缩功能指标〔如射血分数 (EF)和左心室后壁增厚率 (Δ T% )〕明显增加(P<0 .0 1) ;另外 ,对伴舒张功能不全者 ,其 A/ E比值和 A峰也明显下降 (P<0 .0 5 )。上述心功能指标的改善均优于对照组 (P均 <0 .0 5 )。实验组心脏舒张功能的改善似从属于收缩功能的改善。在改善心功能的同时 ,实验组 ANF和 FT3也明显升高 (P<0 .0 5 )。 FT3的提高和心功能改善明显相关 ,而 FT4的提高和心功能改善的关系不大。结论 :WYJXL
Objective:To study the curative effect of Wenyang Jianxin Ling drink(WYJXLD,温阳健心灵口服液) on deficient systolic heart failure.Methods:One hundred and twenty patients were randomly divided into two groups,they were given WYJXLD(trial group) or captopril(control group) respectively for 2 weeks.The cardiac function was evaluated by Doppler echocardiography in all patients and the levels of atrial natriuretic factor(ANF),endothelin(ET) and free triiodothyronine(FT 3),free thyroxine(FT 4) before and after treatment were measured in partial patients separately.The curative effect was judged by the 《Guideprinciple of Traditional Chinese Medicine Treating Congestive Heart Failure》 published by the Public Health Department. Results:Two groups had no difference in effective rate by Radit analyse (67.8% vs.60.0%).The indexes about systolic function such as ejection fraction(EF) and the thickening rate of left ventricular posterior wall were significantly increased in trial group(P<0.01).The A/E ratio and peak Awave were decreased obviously in cases who had diastolic heart failure simultaneously in trial group (P<0.05).The improvement of abovementioned cardiac function indexes in trial group were superior to those in control group (P<0.05).The improvement of the diastolic function seemed to subordinate to the improvement of systolic function. With the improvements of cardiac functions the ANF, FT 3 and FT 4 were obviously increased in trial group.Regarding the thyroid hormone the increase in FT 3 was markedly related to the improvement of cardiac function but the increase in FT 4 was not.Conclusions:WYJXLD has good shortterm curative effect on systolic heart failure in the near future.
