摘要
目的:观察华蟾素肝动脉栓塞和灌注治疗晚期原发性肝癌的临床疗效。方法:晚期不宜手术的原发性肝癌60例,随机分成治疗组(30例)和对照组(30例),治疗组采用左锁骨下动脉穿刺插管,先以华蟾素肝动脉灌注及与碘油混合作肝动脉栓塞后,于左锁骨下埋植导管药盒,连续肝动脉灌注华蟾素15天。对照组采用常规右腹股沟动脉穿刺插管肝动脉栓塞化疗。结果:治疗组完全缓解(CR)3例,部分缓解(PR)18例,稳定(NC)7例,进展(PD)2例总有效率(CR+PR)70.0％;1、2、3年生存率分别为62.4％、33.6％和25.7％。大部分临床症状得到不同程度改善。无明显肝功能急性损伤,部分病例肝功能较前改善,血清肝纤维化指标CA、HA、LN、PCD和Ⅳ-C均有所降低。对照组CR5例,PR18例,NC6例,PD1例,总有效率76.7％;1、2、3年生存率42.6％、24.5％和17.9％。结论:华蟾素肝动脉灌注,在抗肿瘤的同时具有改善肝功能,防治肝纤维化的作用,使生存质量得到提高,生存期延长,尤适宜晚期肝癌肝功能不良患者,值得进一步观察和研究。
objective: To evaluate the way of using hepatic artery injection of Cinobufotian for the treatment of advance hepatocellular carcinoma. Methods: 60 cases of advanced hepatocellular carcinoma were divided into two groups (the treatment group and the control group). The percutaneous trans-arterial implantation of portcatheter system (PCS) through subclavian artery and injection of Cinobufotian and lipiodol or Cinobufotain alone through the PCS were taken in the treatment group,while the routine TACE through femoral in the control group. Results:In treatment group, 3 cases reached complete reaction (CR), 18 cases part reaction (PR), 7 none change (NO and 2 progress development (PD), the efficacy rate (CR + PR) was 70. 0% , and 1, 2 and 3 years survival rate were 62. 4% , 33. 6 % and 25. 7 % respectively. In control group, 5 reached CR, 18 PR, 6 NC and 1 PD. efficacy rate was 76. 7%. 1, 2, 3 years survival rate were 42. 6% , 24. 5% and 17. 9% respectively. There was no remarkable difference in the two groups. Serum CA、HA、 LN、PC Ⅲ and Ⅳ -C decreased and liver functions were improved in treatment group. All clinical symptoms relieved and no adverse effect was seen. Conclusion: Hepatic artery infusion of Cinobufotain has anti-cancer effect, and it can improve liver function and prevent hepatic fibrosis. The outcome of advance HCC may be improved,and this method is suitable for the cases with liver dysfunction. The further observation and research are needed.
