摘要
目的：探讨不能切除的肝癌和肝癌术后复发ＴＡＥ治疗后的并发症的特点。方法：根据行ＴＡＥ前是否曾经肝癌切除手术分Ａ、Ｂ二组比较分析。Ａ组病人３４例，不能或不宜手术切除而行ＴＡＥ治疗，共７７例次。Ｂ组病人１１例，因肝癌根治性切除术后复发，或肝癌姑息性切除术后而行ＴＡＥ，共１６例次。结果：ＴＡＥ术后并发症：Ａ组３例出现肝功能不全，３３次（４２．８％）出现高热，上腹痛以肝区为主（７２．７％）；Ｂ组病人ＴＡＥ后无肝功能不全，２例（１２．５％）出现高热，上腹痛以剑突下为主（６２．５％）。结论：不能或不宜手术切除的肝癌病人ＴＡＥ治疗并发症以肝功能不全和高热为主；肝癌术后复发ＴＡＥ治疗后并发症以剑突下痛为主。两组的差异与使用栓塞物的剂量不同有关。
Objective: The purpose of this study was to investigae the features of complications after transcatheter hepatic artery chemoembolization (TAE) for patients with unresectable primary liver carcinoma (PLC) and for patients with recurrence after hepatectomy. Methods: The patients with TAE treatment in this study was divided into two groups. There was 34 cases in group A, who underwent TAE with 77 times due to unresectable PLC. There was 11 cases in group B, who underwent TAE with 16 times due to recurrence after radical hepatentomy or due to palliative hepatectomy. Results: The main complications after TAE in these 2 groups were as follows. In group A, 3 cases showed hepatic insufficiency after TAE. In group B, there was no case with hepatic insufficiency after TAE. The incidence of high fever (>39℃) was 42.8%(33/77) and 12.5%, respectively in group A and group B. The region of abdomen pain was mainly in right upper abdomen (72.7%) in group A and middle upper abdomen (62.5%). Conclusions: The main complications of TAE were hepatic insufficiency and high fever for patients with unresectable PLC and the pain in the region of upper middle abdomen for patients with recurrence after radical hepatectomy or with palliative hepatectomy.
