摘要
目的研究兔供肝糖原在缺血再灌注期间的变化及其代谢特点,并探讨其意义。方法供体组兔(n=21)均分为三组:A组(禁食24h)、B组(正常喂食)和C组(正常喂食+葡萄糖补充),运用自创的兔肝保存-再灌注模型来观察冷保存、复温和再灌注期间供肝组织中糖原含量、乳酸含量及肝功酶学的变化,并计算缺血期间糖原下降速率。其中B组为正常对照组。结果同B组相比较,禁食24h后,A组肝糖原含量显著下降(15.4mg·g^(-1)±2.6mg·g^1) vs (51.8mg·g^1+6.6mg·g^1),P<0.01,而C组经补充250g·L^1葡萄糖120mL后,肝糖原水平显著提高(63.2mg·g^1±5.8mg·g^1)vs(51.8mg·g~±6.6mg·g^1),P<0.05:经冷存6h。各组糖原水平均显著下降(P<0.05);于复温末,各组肝糖原水平均降至—最低值,彼此相差无显著性(P>0.05),同时意外发现复温期间肝糖原下降速率与冷存末糖原水平呈显著性正相关(P<0.01),于冷存末、各组供肝组织中的乳酸水平均显著升高(P<0.05),于复温末,B,C两组达到最大值,两两间相差具有显著性。于再灌注期间,C组肝功明显好于A,B两组。结论禁食或补充葡萄糖可显著性改变兔肝糖原含量;供肝获取前糖原水平越高,于缺血期间无氧酵解水平越高;复温缺血可能在缺血再灌注损伤中起重要作用。
AIM To evaluate the change and characteristics of glycogen in rabbit donor liver during ischemiareperfusion. METHODS Donor group (n=21) was divided into 3 subgroups randomly: A(n=7): fasting for 24 hours; B (n=7): normal laboratory chew; C(n=7): normal laboratory chew plus glucose supplement intravenously. With the help of self-created animal model for ischemiareperfusion, the levels of glycogen and lactate of liver tissue were measured, and the glycogen-decreasing rate during cold preservation and rewarming period was calculated. RESULTS Compared with group B, the glycogen level of group A after 24-hour fasting decreased significantly (15.4mg·g^(-1)±2.6mg·g^(-1)) vs (51.8mg·g^(-1)±6.6mg·g^1), P<0.01, but that of group C increased markedly (63.2mg·g^(-1)±5.8mg·g^(-1) vs (51.8mg·g^(-1)±6.6mg·g^(-1), P<0.05. After 6-hour cold preservation, the level of glycogen in all groups decreased markedly. At the end of 1-hour rewarming, the levels of glycogen in all groups decreased to the bottom. The glycogen-decreasing rate during rewarming period was significantly positively correlated to the glycogen level at the end of cold preservation. The level of lactate of group C was significantly higher than that of groups A and B during cold preservation and rewarming period. CONCLUSION Fasting or glucose supplement can change the glycogen level of donor liver. The high level of glycogen can provide a high level of anaerobic glycolysis during ischemia period. This study also suggests that rewarming ischemia play an important role in ischemia reperfuaion injury.
