摘要
目的观察高血压及高血压合并糖尿病患者血浆 RAS活性对血浆胰岛素水平的影响。结果 1按血浆胰岛素水平分组 :B组 (>10 <15 m Iu/ L)、C组 (>15 <2 0 m Iu/ L)和 D组 (>2 0 m Iu/ L)及高血压合并糖尿病组患者血浆肾素活性 (PRA)、血管紧张素 (AT )水平明显高于 A组 (<10 m Iu/ L)患者。血糖 /胰岛素 (G/ I)则显著降低。高血压组患者中 ,PRA和 AT 浓度呈现随胰岛素水平升高而增加的趋势 ,但无统计学意义。 2按 PRA分组 :将高血压患者按 PRA分成三组 ,a组 (PRA<1.0 ng/ m l/ h) ;b组 (PRA>1.0 <1.5 ng/ ml/ h)和 c组 (PRA>1.5ng/ ml/ h)。结果显示 ,b组和 c组的胰岛素水平明显高于 a组 ,并且胰岛素浓度随 PRA活性增加而明显升高。结论高血压患者 RAS活性增加与血浆胰岛素浓度增加密切相关 ,RAS活性增加可能是胰岛素抵抗 (IR)的原因之一。
Objective To observe the effects of renin angiotensin system on the plasma insulin levels in patients with hypertension and hypertension complicated with diabetic mellitus.Results ① The patients were subgrouped on plasma insulin levels: Compared with subgroup A (insulin levels was <10 mIu/L),the plasma renin activity (PRA) and angiotensin Ⅱ(ATⅡ) were significantly increased, and the glucose/insulin ratio (G/I) were decreased significantly in subgroup B (insulin levels was>10,<15 mIu/L), subgroup C(>15,<20 mIu/L), subgroup D(>20 mIu/L) and in the group complicated with type 2 diabetic mellitus. ② According to PRA, we subdivided the hypertensive patents into three subgroup, group a (PRA was <0 1 ng/ml/h); group b (>1 0,<1 5 ng/ml/h); group c(>1 2 ng/ml/h). The results showed that plasma insulin levels were higher in group b and c than that in group a and rose significantly with the increase in RAS.Conclusion The increased activity of RAS is correlated with the hyperinsulinemia and may be one of the causes of insulin resistance(IR).
