摘要
目的 :探讨 2型糖尿病周围神经病变 (DPN)患者血清抗神经节苷脂抗体 (anti- GS- ab)与细胞免疫之间的关系。方法 :受试者分为三组 :糖尿病无神经病变组 (DM组 ) ,DPN组 ,正常对照组 (N组 ) ,每组 30人。采用美国 Nicolet Viking 型肌电图仪测定神经传导速度 ,固相酶免法测定各组 anti- GS- Ig M- ab、 anti- GS- Ig G- ab,同时观察 T淋巴细胞亚群、白介素 (interleukin,IL) IL- 1、IL- 2、肿瘤坏死因子 (tum or necrosis factor,TNF- α)、一氧化氮 (NO)。结果 :DPN组 anti- GS-ab水平明显高于 N组及 DM组 ,T淋巴细胞亚群 CD+ 4 ,CD+ 8,IL - 2值水平明显下降 ,IL - 1β、 TNF-α显著增高。逐步回归分析 ,DPN组对 anti- GS- ab有显著作用的因素依次为 DPN的临床分级 (DPNC) ,NO,IL- 1β。结论 :anti- GS- ab的检测可作为一项了解 DPN自身免疫状况的免疫学指标 ,对 DPN的早期诊断 ,病情及疗效判断有参考价值。
Objective:To investigate the relationship between anti GS ab and cell immunity in diabetic polyneuropathy(DPN) Methods:The patients were divided into 3 groups:NIDDM patients without DPN (group DM),NIDDM with DPN(group DPN),health people(group N) Each group has 30 people We use American Nicolet Viking IV electromyography to measure neural conduction rate and immobilired enzyme immunoassay to measure anti GS IgM ab,anti GS IgG ab,and observe T lymphocyte subgroup,interleukin 1(IL 1),IL 2,tumor necrosis factor(TNF a) and NO Results:In group DPN,anti GS ab level is higher than group N and group DM,T lymphocyte subgroup CD +4 ,CD +8 ,but IL 2 level decreased obviously,while IL 1β,TNF α increased significantly Regulation analysis was done in group DPN influent factors of anti GS ab are DPNC,NO,IL 1β in sequence Conclusion:[WT5,5”BZ]Measurement of anti GS ab as an immunological signal to explore autoimmunity condition is valuable for early diagnosis of DPN,and judgment of clinical condition and remedy effect
