摘要
目的 评价老年全膀胱切除术、尿流改道术的安全性 ,探讨适合老年人的尿流改道术式。方法 回顾性分析 58例老年膀胱癌患者施行全膀胱切除和尿流改道术的临床资料和随访结果。结果 58例患者中 ,手术后发生并发症 2 7例 ,1例死于术后腹腔漏尿伴发感染 ,1 4例改良的可控性尿流改道术 ,1 3例控尿满意。结论 全膀胱切除术和尿流改道术对老年膀胱癌患者安全可行 ,自行改良的可控性尿流改道术更适合于老年患者。
? Objective To study the security of cystectomy and urinary diversion in elderly patients.Methods The clinical materials of 58 patients over 60 years old who underwent cystectomy and urinary diversion during 1986～1999 were reviewed and analysed including mortality,complication,duration of hospitalization.Results The surgical mortality for the elderly patients was 1%.The incidence of complications in elderly was 46%.The duration of surgery was 6 9 hours.the duration of hospitalization of elderly was 27 days,no significant difference with non aged group.Conclusions It is safe for elderly patients to select cystectomy and urinary diversion Modified indiana pouch with cecum segment and an unaltered,in situ appendiceal conduit is more adaptable to elderly patients. 〔
