摘要
臀部纤维束带和臀中肌瘢痕化是妨碍髋关节内收、内旋的主要原因,彻底松解这些组织是手术成功的关键。应用“Z”形手术治疗臀肌筋膜挛缩症45例,术后10天均能步行,其中43例能双腿并拢完全下蹲;2例只能屈髋90°。31例随访1～26个月,30例步态正常,并腿下蹲试验阴性,髋关节内收大于20°,臀肌肌力正常,1例步态正常,并腿下蹲屈髋90～130°,髋关节内收0～20°,臀肌肌力正常。其中29例手术切口出现明显增生性瘢痕。
45 case of contracture of gluteal muscular fascia were treated with Z plastic surgery. Excellent result were obtained. Fibrous band in buttock and middle gluteal muscular cicatrization is main factors to limit the range of abduction andiintorsion of the hip joint. Release these fibrous tissue entirely is essential to the succes of the operation. reconstruction of the contact between the greatest gluteal muscle, gluteal fascia and iliotibial tract, and early functional exercise are much more beneficial for earlier recovery of the hip function. This kind of Z plastic surgery is simple, safe and convenient to operate. After operation, the rehabilitation of the lower limb is got rapidly. We think that it is one method of the best treatment of the disease.
