摘要
目的探讨臀大肌止点上移松解术治疗重型臀肌挛缩症的临床价值。方法重型臀肌挛缩症患者38例行臀大肌止点上移松解术,观察临床效果。结果平均随访29个月,38例患者术后功能改善满意;驼背与骨盆后倾消失35例(97.2%),步态基本恢复正常31例(86.1%),轻度摇摆步态4例(11.1%),1例仍有明显跛行(2.8%);与术前相比,中立位平均屈髋改善94°;在屈髋90°时,髋内收功能平均改善75;°双髋外展肌力正常者32例(88.9%),单侧外展肌力较差者3例(8.3%),双侧外展肌力较差者1例(2.8%)。结论臀大肌止点上移松解术治疗重型臀肌挛缩症,疗效满意。
Objective To investigate the clinical value of operation of upwards shift of the insertion of gluteus maximus and release of gluteus medius treatment for severe gluteus maximus contracture. Methods Thirty eigtht patients were diagnosed as severe gluteus maximus contracture treated in our hospital between 2004 and 2007. All patients had undertaken operation of upwards shift of the insertion of gluteus maximus and release of gluteus medius. Results Thirty six patients were followed up from 12 -36 months （mean,29 months）. All patients were satisfied with their postoperative improvements. 35 （97.2%） patients were free from kyphosis and pelvic retroversion ;31 （86. 1% ） patients almost recovered to normal gait ; 4 （ 11. 1% ） patients remained mild swinging gait ; 1 （ 2. 8% ） patient still crippled. Comparing with preoperative condition,the hip flexion was improved with an average 94~,when patients were on hip abduction 90°,hip abduction was improved with an average 75°.3（8.3% ） patients had unilateral abduction weakness. 1（2.8% ） patients had bilateral abduction weakness. Conclusion Operation of upwards shift of the nsertion of gluteus maximus and release of gluteus medius may gain satisfactory effects for patient with severe gluteus maximus contracture.
