摘要
目的探讨避孕方式与宫颈人乳头瘤病毒(HPV)感染的关系,分析节育环对患者肿瘤坏死因子(TNF-α)的影响。方法 2011年6月至2013年6月收治的2 576例患者作为研究对象,其中宫颈HPV感染的患者120例,统计所有避孕方式,对比分析不同避孕方法致患者宫颈HPV感染的差异。采用酶联免疫吸附试验(ELISA)检测宫内节育器的50例女性(健康对照组)和无宫内节育器的50例女性(研究组)的宫颈黏液中TNF-α。结果宫内节育器组感染比例最低,感染率1.92%(10/542),与宫内节育器组比较,避孕套组、口服避孕药组HPV感染病例均高于宫内节育器组,差异有统计学意义(P<0.05),输卵管结扎组HPV感染病例高于口服避孕药组,差异有统计学意义(P<0.05)。研究组宫颈黏液的TNF-α为(88.94±35.78)pg/mL,健康对照组为(14.67±6.31)pg/mL,研究组明显高于健康对照组,差异有统计学意义(P<0.05),但不同材质的宫内节育器亚组间TNF-α比较,差异无统计学意义(P>0.05)。结论采取宫内节育器避孕方式的女性HPV感染比例较小,但宫颈黏液TNF-α水平较高,节育器可能通过改善局部免疫状态起到预防HPV感染的作用。
Objective To investigate the relations of contraception methods and cervical human papillomavirus （HPV） infection ,and analyzes the impact of IUD on patients′s TNF-α.Methods From June 2011 to June 2013 , 2 576 patients were treated as research subjects ,including cervical HPV infection in 120 patients ,all methods of con-traception were statisticed ,different cervical HPV infection Infection rates in patients with dfferent contraceptive methods were compared .The TNF-αwere checked by ELISA on 50 cases IUD patients（study group）and 50 cases no IUD women（control group） .Results IUD group had the lowest proportion infection rate ,the HPV infection rate was 1 .92% （10/542） ,compared with the IUD ,condoms group ,oral contraceptives group（P〈0 .05） ,HPV infection rate in the pill group were higher than that of the intrauterine device group ,the difference was statistically significant （P〈0 .05） ,tubal ligation HPV infection rate is higher than that of pill group ,difference was statistically significant （P〈0 .05） .TNF-αin study Group cervical mucus was（88 .94 ± 35 .78） pg/mL ,that of control group was（14 .67±6 .31） pg/mL ,the study group was significantly higher（P〈0 .05） ,but there was no significant difference in different materials IUD of control subgroups（P〉0 .05） .Conclusion The women with IUD contraception has a small propor-tion HPV infection ,but the TNF-αof cervical mucus is higher .It may play a role in the prevention of HPV infection by improving the local immune status .
