摘要
目的 分析胰腺动脉在选择性腹部动脉造影中显影的特点及规律 ,为在临床动脉灌注治疗重症急性胰腺炎等胰腺疾病时动脉的选择提供依据。方法 将我院 1981年至 1999年 8年间 2 0 3例经腹部动脉造影的资料进行分类、整理 ,统计出胰腺主要供血动脉的显影率。结果 119例选择性腹腔干造影中 ,胰十二指肠上动脉 (SPD .a)显影的阳性率为 3 2 % ,胰十二指肠前、后动脉弓显影率为16% ,胰背动脉显影率为 2 7 7% ,仅有 1例胰十二指肠下动脉 (IPD .a)显影 ;5 5例选择性肠系膜上动脉(SM .a)造影中 ,胰腺动脉中只有IPD .a显影 ,显影率为 4 0 % ;18例腹腔干 SM .a联合造影中 ,SPD .a、胰十二指肠前、后动脉弓、胰背动脉、IPD .a的显影率分别为 5 5 6%、2 7 8%、61 1%、2 7 8%。结论 根据本组资料 ,采用动脉联合造影胰腺动脉的显影率比单一动脉造影高。
Objective To analyze the characteristics and rules of development of pancreatic arteries in selective arteriography via abdominal arteries to provide basis for arterial choice in arterial infusion (AI) for treatment of pancreatic diseases such as severe acute pancreatitis etc. Methods The data of 203 cases undergoing selective arteriography via abdominal arteries in our hospital from May 1981 to August 1999 were classified and sorted to calculate the positive rates of main arteries supplying the pancreas. Results In 119 cases undergoing selective celiac angiography, positive rates of the superior pancreaticoduodenal artery (SPD.a), anterior and posterior pancreaticoduodenal arcades and dorsal pancreatic artery were 32%, 16% and 27.7%, respectively. However, only one inferior pancreaticoduodenal artery (IPD.a) was developed. In 55 cases undergoing superior mesenteric angiography, only IPD.a was developed and its positive rate was 40%. In 18 cases receiving celiac and SM.a co-angiography, positive rates of SPD.a, two arcades, dorsal artery and IPD.a were 55.6%, 27.8%, 61.1% and 27.8%, respectively. Conclusions According to these results, we can conclude that the positive rate of pancreatic arteries by co-angiography is higher than that by single-artery angiography.
