超声弹性成像应变率参数在甲状腺良恶性结节鉴别诊断中的应用价值 被引量:26

Application value of ultrasound elastography strain rate parameter in the differential diagnosis of benign and malignant thyroid nodules

摘要 目的探讨超声弹性成像应变率参数在甲状腺良恶性肿块鉴别诊断中的应用价值。方法对152例患者172个甲状腺肿块进行超声弹性成像检查,计算肿块与周围正常组织的应变率比值(SR比值),绘制ROC曲线图,确定区分甲状腺肿块良恶性应变率参数的临界点。结果 172个病灶中良性肿块65个,SR比值平均为2.32±0.19;恶性肿块107个,SR比值平均为8.79±0.42,二者差异有统计学意义(P<0.01)。根据良恶性肿块的SR比值绘制ROC曲线,曲线下面积为0.897±0.028,分析得出良恶性结节的SR比值临界点为3.01,其对应敏感性为95.59%,特异性为88.63%,约登指数为0.84。结论超声弹性成像应变率参数可作为甲状腺良恶性结节的鉴别诊断依据,在临床诊断中具有较高的应用价值。 Objective To explore the application value of ultrasound elastography strain rate in the differential diagnosis of benign and malignant thyroid nodules. Methods Ultrasound elastography detection was performed for 152 patients with 172 thyroid masses. Then the ratio between the strain rate(SR) of mass and the surrounding normal tissue was calculated,the ROC graph was drawn, and finally the critical point distinguishing the benign and malignant thyroid nodules strain rate parameter was defined. Results In the 172 focuses, there were 65 benign masses with the average SR ratio (2. 32 ± 0. 19), while 107 malignant masses with the average SR ratio (8. 79 ±0. 42). There were statistical differences between them(P 〈0. 01 ). The ROC curve was drawn according to the SR ratio of the benign and malignant mass. The area under the curve was 0. 897 ± 0. 028. After analysis, it was concluded that the critical point of ratio of the benign and malignant thyroid nodules strain rate was 3.01, the corresponding sensitivity was 95.59%, the specificity was 88.63%, and the Youden index was 0. 68. Conclusions Ultrasound elastography strain rate parameter can be regarded as the differential diagnosis basis for benign and malignant thyroid nodules, and has high application value in clinical diagnosis.
