摘要
目的:利用三维超声评估不同分娩方式对肛提肌形态及完整性的影响。方法:选择2013年3~10月产后1~3个月复诊的初产妇180例,使用三维经会阴超声进行检查,TUI成像模式显示肛提肌裂孔最小平面,观察肛提肌的形态及损伤情况。结果:172例初产妇获得完整数据:阴道分娩90例(顺产81例,器械助产9例),剖宫产术后82例(选择性剖宫产60例,紧急剖宫产22例)。阴道分娩后的盆底超声测量指标除了肛提肌厚度之外均高于剖宫产术后组,差异有统计学意义(P〈0.05)。肛提肌损伤与阴道分娩有显著的关系,阴道分娩19例有肛提肌损伤(顺产16例,器械助产3例)明显高于剖宫产术后组(2例均为紧急剖宫产),差异有统计学意义(P〈0.05)。结论:阴道分娩的方式较剖宫产明显改变了肛提肌的形态及完整性。
Objective: To explore the effects of different delivery modes evaluated by three - dimensional ultrasound on morphology and integrity of levator ani muscle after childbirth. Methods: A total of 180 primiparous women who went to the hospital for subsequent visit at 1 - 3 months after childbirth from March to October in 2013 were selected, transvaginal three - dimensional ultrasound was used to examine the women, the minimal hiatal dimension of levator ani muscle was displayed under TUT imaging mode, the morphology and damage of levator ani muscle were observed. Results: Intact data were obtained from 172 primiparous women: 90 women adopted vaginal delivery (81 women of spontaneous birth and 9 women of instrument midwifery), 82 women adopted cesarean section (60 women of elective cesarean sec- tion and 22 women of emergency cesarean section) ; except the thickness of levator ani muscle, the pelvic ultrasonic indexes in vaginal deliv- ery group were statistically significantly higher than those in cesarean section group ( P 〈 O. 05 ) ; levator ani muscle injury was significantly correlated with vaginal delivery; 19 women in vaginal delivery group were found with levator ani muscle injury (16 women of spontaneous birth and 3 women of instrument midwifery), which was statistically significantly higher than those in cesarean section group (2 women of emergency cesarean section) (P 〈 0. 05) . Conclusion: Compared with cesarean section, vaginal delivery change the morphology and integrity of levator ani muscle significantly.
