经会阴三维超声评价女性盆底肌肉形态及功能 被引量:8

Three-dimensional transperineal ultrasound in assessment of pelvic floor muscle anatomy and function

摘要 目的应用经会阴三维超声测量年轻未育女性的耻骨内脏肌宽度及肛提肌裂孔大小,探讨女性盆底的形态及功能。方法对100例18~31岁年轻未育女性在安静状态下、最大瓦氏动作及提肛动作时采集盆底经会阴三维超声图像,脱机分析,分别测量肛提肌裂孔的前后径、左右径、面积及耻骨内脏肌的宽度,并通过计算组内相关系数评估观察者之间的一致性。结果肛提肌裂孔呈菱形,两侧耻骨内脏肌基本对称,呈带状高回声,在肛直肠角后方形成U型襻。安静状态下肛提肌裂孔面积(11.15±1.70)cm2,最大瓦氏动作时增大至(14.35±2.43)cm2,而提肛动作时缩小至(9.20±1.46)cm2。两名观察者测量安静状态及提肛动作时的肛提肌裂孔大小及不同状态下耻骨内脏肌宽度一致性较好,但在最大瓦氏动作时测量肛提肌裂孔大小的一致性稍差。结论经会阴三维超声能直观观察盆底耻骨内脏肌的宽度及肛提肌裂孔的动态变化。 Objective To explore the pelvic floor anatomy and function of young nulliparous women by measuring the pubovisceral muscle and levator hiatus under rest, Valsalva maneuver and muscle contraction with three-dimensional (3D) transperineal ultrasound. Methods One hundred young nulliparous women aged 18 31 years were enrolled. 3D ultra-sound volumes were acquired under rest and during Valsalva and muscle contraction, and then were analyzed offline. The dimensions of pubovisceral muscle and [evator hiatus were measured. Interclass correlation coefficients were calculated to e- valuate the consistency of the data. Results The shape of levator hiatus was rhombic. Bilateral pubovisceral muscle was hyperechoic and symmetrical, interweaved into the U-shape loop in the rear of anorectum. Under rest condition, the area of levator hiatus was (11.15 ± 1.70)cm^2 , which enlarged to (14.35 4±2.43)cm^2 during Valsalva maneuver, but reduced to (9. 204±1.46)cm^2 during contraction. The repeatability was good under rest and during muscle contraction, but was rela-tively poor during Valsalva maneuver. Conclusion 3D transperineal ultrasound is a reliable method for obtaining visual in-formation of functional anatomy of the levator hiatus and pubovisceral muscle.
