摘要
目的分析双眼外直肌超常量后徙治疗共同性外斜视的安全性及长期疗效。方法回顾分析第二炮兵总医院眼科就诊的46例共同性外斜视患者，按遮盖24h后最大手术量设计，双眼外直肌后徙5～14mm。所有患者行详细相关检查。术后随访2-3年，分析近期和远期手术效果。结果术后1周：31例正位，斜视度为＋8^△～-8^△；有9例轻度过矫，6例明显过矫，其中1例需要再手术。术后3个月，37例正位，斜视度为＋8^△～8^△；有5例轻度过矫，4例轻度欠矫。术后2-3年，38例正位，斜视度为＋8^△～8^△，有5例轻度欠矫，3例明显欠矫。眼球运动正常。结论双眼外直肌超常量后徙治疗共同性外斜视长期手术效果稳定，安全性高。
Objective To observe the effect of the long-term results and security with the super-normal bilateral rectus muscle recession for concomitant exotropia. Methods The pre-and post-oper- ative alignment of 46 patients with concomitant exotropia performed by supemormal bilateral rectus muscle recession was analyzed retrospectively. The amount of recession was between 5-14mm per lat-eral rectus muscle. The angle of exotropia was measured by the prism and cover test. The amount of surgery performed was based on the distance exotropic deviation after being occluded for 24 hours. All detailed ophthalmological examinations were made in every patient preoperatively and post-operatively. The follow-up period ranged from 24 to 36 months （average 27 months）. Long-term and short-term surgical outcomes were analyzed for all patients included in the study. Results After 1 week of post-operation of follow-up, 31 patients who underwent supernormal bilateral rectus muscle recession were normtropia （-8^△≤deviation〈8^△）, 9 patients were overcorrected lightly （8^△≤deviation≤ 15A）, 6 patients were overcorrected （15^△〈deviation ≤25^△）, 1 patient needed a re-operation. After 3 months of post-operation of follow-up, 37 patients who underwent supernormal bilateral rectus mus- cle recession were normtropia （-8^△≤deviation≤8A）, 5 patients were overcorrected lightly （8^△〈devia- tion≤15^△）, 4 patients were undercorrected lightly （-15^△≤deviation≤-8^△）. After average 27 months （be-tween 24 months and 3 years） of post-operation of follow-up, 38 patients who underwent supemor-mal bilateral rectus muscle recession including loose suture were normtropia （-8^△〈deviation≤8^△）. 5 patients were undercorrected lightly （-8^△≤deviation≤-15^△）, 3 patients were undercorrected obviously （-8^△ deviation）. All patients had full ductions and versions postoperatively. Conclusions Supernor-mal bilateral rectus muscle recession in concomitant exotropia has good and stable long-term effects and security.
