摘要
目的观察中医综合疗法联合西药治疗重症急性胰腺炎的临床疗效。方法将96例重症急性胰腺炎患者随机分为治疗组及对照组,每组48例。两组均予液体复苏、抗酸、抑酶等常规治疗,治疗组加用清胰解痉方口服及大黄煎剂灌肠治疗。两组疗程均为3周,观察主要临床症状(腹痛、腹胀、无肛门排气)的缓解时间,相关血液生化指标(血清胰淀粉酶、血白细胞、血钙离子)恢复正常水平的时间,并比较死亡率、并发症发生率情况。结果①组间治疗后比较,腹痛、腹胀的缓解及肛门排气的时间差异有统计学意义(P<0.05);血清胰淀粉酶、WBC、Ca2+恢复正常的时间差异有统计学意义(P<0.05)。②治疗结束后,治疗组、对照组的死亡率分别为8.3%、18.8%,差异有统计学意义(P<0.05);治疗组、对照组的并发症发生率分别为10.4%、25.0%,差异有统计学意义(P<0.05)。结论中医综合疗法联合西药治疗重症急性胰腺炎,能够明显改善患者的临床症状,且起效较快,死亡率、并发症发生率较低。
Objective To observe the clinical efficacy of TCM comprehensive therapy and western medicine for the treatment of acute severe pancreatitis. Methods Ninety-sixty cases with acute severe pancreatitis were randomized into treatment group and control group, with 48 cases in each group. Both of the groups were treated by conventional therapy. Additionally, treatment group was treated with ＂Qingyi Jiejing Formula＂ orally and ＂Dahuang Decoction＂ enema, with the course of 3 weeks. The remission of clinical symptoms, reablement of blood biochemical indexes were observed, and the death rate and complications were detected. Results （1） After treatment, there were significant differences in alleviation of abdominal pain and abdominal distention, and evacuating time between the two groups （ P 〈 0.05 ） ; significant differences were found in time of amylase, WBC and Ca^2 ＋ reablement between the two groups （ P 〈 0.05 ）. （2） After treatment, the death rate was 8.3% in treatment group and 18.8% in control group, with a difference between the two groups （P 〈 0.05）. The occurrence rate of complication was 10.4% in treatment group and 25.0% in control group, with a difference between the two groups （P 〈 0.05）. Conclusion Combined TCM comprehensive therapy with western medicine can improve the clioical symptoms rapidly, with low mortality rate and complication rate.
