摘要
目的观察套圈法回复肛垫治疗环状混合痔的效果。方法选取2011年8月一2013年2月治疗的环状混合痔患者72例，随机分为观察组和对照组各36例。观察组给予套圈法回复肛垫治疗，对照组采用外剥内扎法治疗。对比两组疗效、手术时间、出血量、创面愈合时间、住院时间及术后疼痛、肛缘水肿、肛门坠胀、尿潴留情况。计量资料采用t检验，计数资料采用矿检验，P〈0．05为差异有统计学意义。结果两组总有效率、手术时间比较差异无统计学意义（t=1．197，P〉O．05），观察组出血量、创面愈合时间、住院时间[（8．8±4．3）ml、（7．8±2．3）、（6．8±1．9）d]与对照组[（22．3±6．4）ml、（23．6±3．3）、（13．8±4．3）d]比较差异均有统计学意义（t=10．776、23．568、8．934，均P〈0．05）。观察组疼痛0～Ⅱ级分别为20、16、0例，对照组分别为2、27、7例；观察组肛缘水肿0～Ⅱ级分别为21、15、0例，对照组分别为6、20、10例；观察组肛门坠胀0～Ⅱ级分别为14、22、0例，对照组分别为5、26、5例；观察组尿潴留0例，对照组4例。两组比较差异均有统计学意义（X^2=4．494、4．265、2．953、4．235，均P〈0．05）。结论套圈法回复肛垫治疗环状混合痔效果肯定，手术创伤小，恢复快。
Objective To investigate the effects of looped suture for the restoration of anal cushion in treating circumferential mixed hemorrhoids.Methods 72 cases of circumferential mixed hemorrhoids treated from Aug.2011 to Feb.2013 were randomly divided into two groups：observafion group and control group,36 cases for each group.Observation group received looped suture for the restoration of anal cushion,while control group underwent external dissection and internal ligation.Both groups were compared for clinical efficacy,operative time,bleeding amount,wound healing time,hospital stay,as well as postoperative pain,swelling of anal margin,beating down sensation,urinary retention.Measurement data were processed by t-test,count data by x2 test,and the result of P〈0.05 was considered statistically significant.Results No significant difference was found between the two groups in total effective rate and duration of operation（t=1.197,P〉0.05）.The amount of bleeding,wound healing time,hospital stay were （8.8± 4.3）ml,（7.8± 2.3） and （6.8± 1.9）d,respectively in observation group,which were significantly different from those in control group[（22.3 ±6.4）ml, （23.6± 3.3）,（13.8 ± 4.3）d],（t=10.776,23.568,8.934,P〈0.05）.In observation group,20 cases with pain of scale 0,16 with scale 1 and none with scale 2;in control group,there were 2 cases with pain of scale 0,27 with pain of scale 1 and 7 with pain of scale 2.In observation group,there were 21 cases with swelling of anal margin of scale 0,15 with scale 1 and none with scale 2;in control group,there were 6 cases with swelling of anal margin of scale 0,20 with scale 1 and l0 with scale 2.In observation group,there were 14 cases with anal bearing down sensation of scale 0,22 with scale 1 and none with scale 2;in control group,there were 5 cases with anal bearing down sensation of scale 0,26 with scale 1 and 5 with scale 2.There was significant difference between the two groups（X^2=4.494,4.265,2.953,4.235,P〈0.05）.Conclusion Looped suture for the restoration of anal cushion is effective for circumferential mixed hemorrhoids,with less surgical invasion and rapid recovery.
