摘要
目的观察自血疗法对泛发性皮肤瘙痒症患者的临床疗效及其对辅助性T细胞1(Th1)/Th2细胞因子平衡的影响。方法将66例患者按完全随机化法分为治疗组和对照组各33例。治疗组患者在口服氯雷他定基础上加用自血疗法,对照组患者仅口服氯雷他定。两组均治疗4周。观察两组症状变化,采用双抗夹心酶联免疫吸附试验法检测两组患者治疗前后Th1细胞因子[干扰素-γ(IFN-γ)、白介素-2(IL-2)]与Th2细胞因子(IL-4、IL-10)水平变化。结果治疗后两组患者症状均有好转,但治疗组明显优于对照组,差异有统计学意义(P<0.05);治疗组治疗后IFN-γ与IL-2水平较治疗前明显降低,差异有统计学意义(P<0.05),且低于对照组治疗后,差异有统计学意义(P<0.05),治疗组治疗后IL-4与IL-10水平较治疗前明显升高,差异有统计学意义(P<0.05),且高于对照组治疗后,差异有统计学意义(P<0.05)。结论自血疗法对泛发性皮肤瘙痒症有良好的疗效,可能是通过调节Th1/Th2细胞因子水平,改善了免疫状态而达到治疗目的。
Objective To observe the clinical effect of autohemotherapy on generalized pruritus patients and its influ-ence on T helper cell（Th）1/TH2 cytokines balance. Methods Totally 66 patients with generalized pruritus were equally random-ized into two groups：the treatment group and the control group. The treatment group was added with autohemotherapy on the basis of oral loratadine,while the control group only received laratadine orally. The treatment course of both groups was 4 weeks. The symptomatic changes were observed in both groups. The double-antibody sandwich enzyme linked immunosorbent assay was adopted to detect the changes of Th1 cytokines[interferon（IFN）-γand interleukin（IL）-2] level and Th2 cytokines（IL-4 and IL-10） level before and after treatment in the two groups. Results The symptoms in both groups were improved ,but the treatment group was superior to the control group obviously with statistically significant difference （P〈0.05）. Levels of IFN-γand IL-2 in the treat-ment group after treatment reduced more obviously than those before treatment with statistically significant difference （P〈0.05）, and the levels of IFN-γand IL-2 in the treatment group after treatment were lower than those in the control group with statistically significant difference（P〈0.05）;Levels of IL-4 and IL-10 in the treatment after treatment increased more prominently than those before treatment with statistically significant difference （P〈0.05）,and levels of IL-4 and IL-10 in the treatment after treatment were higher than those in the control group with statistically significant difference （P〈0.05）. Conclusion Autohemotherapy is effective in curing generalized pruritis,it can improve the immune state by regulating Th1/Th2 cytokines level to achieve the medical purpose.
