摘要
目的：探讨研究并观察十二指肠溃疡急性穿孔时选择不同术式的临床治疗效果情况。方法分析本院2003年1月~2012年2月收治的529例十二指肠溃疡急性穿孔患者临床资料,依据治疗方式的不同进行临床分组,穿孔修补术组355例,胃大部分切除术组174例。结果穿孔修补术组手术时间、术中出血量、下床活动时间、胃肠功能恢复时间、住院时间均低于胃大部分切除术组,穿孔修补术组术后溃疡复发和返流性胃炎均低于胃大部分切除术组,P〈0.05,差异均有统计学意义。结论十二指肠溃疡急性穿孔时选择穿孔修补术创伤较小,值得临床借鉴应用。
Objective To explore clinical treatment effect of different surgical method in acute perforation of duodenal ulcer.Methods To analyze clinical data of 782 cases acute perforation of duodenal ulcer patients in our hospital from January 2003 to February 2012, which were divided into different groups, perforation repair group 355 cases, subtotal gastrectomy group 174 cases and non operation treatment group of 253 cases. Results The operation time, intraoperative blood loss, time to ambulation, recovery of gastrointestinal function, hospitalization time of perforation repair group were lower than subtotal gastrectomy group, the postoperative recurrence of ulcer and gastritis of perforation repair group were lower than subtotal gastrectomy group,P〈0.05, the difference were statistical significance.Conclusion The acute perforation of duodenal ulcer perforation repair wounds smaller selection, which is worth clinical promotion.
