摘要
目的探讨细胞块切片免疫组化染色应用于胸腔积液病理诊断的临床效果。方法选择本院收治的60例胸腔积液患者作为研究对象,分别采用细胞块切片免疫组化染色检测和常规细胞涂片检测,检测患者的CK7、CD15、MOC-31、BerEP4、TTF-1、CEA、Calretinin、WT-1、CK5/6、Desmin等指标表达,对鉴别诊断肺腺癌和间皮病变的相关免疫标记组化进行筛选。结果两组胸腔积液的阳性检出率相比,P<0.05。细胞块切片免疫组化染色检测分型诊断恶性胸腔积液的符合率高达96.67%,经过组合性抗体检测发现,间皮病变呈现Calretinin、WT-1、CK5/6、Desmin指标高表达,而肺腺癌则呈现CK7、CD15、MOC-31、BerEP4、TTF-1、CEA指标高表达,显示细胞块切片免疫组化染色检测明显优于常规细胞涂片检测(P<0.01)。结论在适当的抗体组合下,胸腔积液细胞块切片结合免疫组化染色检测对肺腺癌和间皮瘤等恶性胸腔积液具有非常的病理鉴别诊断效果,值得临床推广。
Objective To explore the clinical effect of cell block slice immunohistochemical staining was applied to pathologic diagnosis of pleural effusion. Methods Selected 60 patients with pleural effusion were treated at our hospital as the research object, the cell block slice immunohistochemical staining were used respectively to test and convention- al smear test, the testing of patient CK7,CD15,MOC-31,BerEP4, TFF-1,CEA,Calretinin,WT-1, CK5/6, Desmin index expression, such as for differential diagnosis between skin lesions of lung adenocarcinoma and filtered immune markers related to network stations. Results The two positive rate of pleural effusion compared, P〈0.05. Cell block sections im- munohistochemical staining typing diagnosis of malignant pleural effusion compliance rate of 96.67%, after a combina- tion of antibodies detected, mesothelial lesions showed Calretinin, WT-1, CK5/6, Desmin indicators of high expression the lung cancer was showing CK7, CD15, MOC-31, BerEP4, TTF-1, CEA indicators of high expression, indicating cell block sections immunohistochemical staining was significantly better than conventional smear test (P 〈0.01). Conclu- sion The under the appropriate combination of antibody, pleural effusion cells block biopsy combined with immunohis- tochemical staining detection for such as lung adenocareinoma and mesothelioma pathological differential diagnosis of malignant pleural effusion is very effect, worth clinical promotion.
