摘要
目的 探讨苯那普利 (benazepril)对糖尿病肾病 (DN)患者肾脏产生及尿液排泌内皮素 - 1(Et- 1)的影响。方法 通过放射免疫方法测定 5 4例糖尿病 (DM)患者血浆 Et- 1浓度 (PEt)及 2 4小时尿 Et- 1排泌量 (UEt V) ,其中 39例糖尿病肾病 (DN)患者观察苯那普利治疗前后 PEt及 UEt V的变化。结果 糖尿病患者 UEt V、PEt均显著增加 (P<0 .0 5 ) ;DN患者较无肾病之糖尿病 (DNM) UEt V更高 (P<0 .0 5 ) ;伴糖尿病视网膜病变及高血压较之不伴有 DN的患者UEt V更高 (P<0 .0 5 ) ;苯那普利治疗两个月后 ,DN患者 UEt V较治疗前显著下降 (P<0 .0 1) ;UEt V与尿白蛋白排泄率及血糖化血红蛋白无相关 ;UEt V与尿 β2 微球蛋白 (β2 MG)及糖尿病病程正相关。结论 DN患者肾脏受损产生 Et- 1致尿液排泌 Et- 1增多 ,苯那普利可能抑制肾脏产生 Et- 1致尿液排泌 Et- 1减少 ,因而对 DN患者肾脏起着一定保护作用 ,为今后进一步探索临床治疗 DN提供一个可行的途径。
Objective To investigate the effects of benazepril on urinary excretion of endothelin 1(Et 1) and the level of plasmic Et 1 (PEt) in patients with diabetes mellitus (DM) and diabetic nephropathy(DN).Methods PEt and 24 hours urinary excretion of Et 1 (UEtV) in 54 patients with DM and 39 patients with DN before and after taking benazepril (7.5~12.5mg/d) for two months were measured using radioimmunoassay method.Results PEt and UEtV in patients with DM were significantly higher than in normal control group ( P <0.05).UEtV in patients with DN was significantly higher than in patients with DM without nephropathy.UEtV in patients with diabetic retinopathy(DR) or hypertension were significantly higher than in patients without DR or hypertension.UEtV in patients with DN after taking benazepril was significantly lower than before that.UEtV in patients with DN was positive correlated with 24 hours urinary excretion of β 2 MG and diabetic development.Conclusion The level of plasmic Et 1 and the urinary excretion of Et 1 increased in patients with DN,so that it would be a marker for the renal injury in DN.Benazepril could inhibit production of Et 1 in patients with DN,which could play a role in protecting the renal function in patients with DN.Benazepril is an effective drug for the treatment of patients with DN.
