摘要
目的 探讨细胞生长因子与一氧化氮在 2型糖尿病 (DM)视网膜病变 (DR)发病中的作用。方法 应用竞争性亲和素 -生物素酶联免疫吸附法和分光光度法检测 74例 DM患者血管内皮生长因子 (VEGF)、γ干扰素 (IFN- γ)和一氧化氮 (NO)的血浆含量 ,分析它们与糖化血红蛋白 (GHb A1c)、空腹血糖 (FBG)、肌酐 (Cr)的关系 ,同时以 30例健康人作为对照。结果 (1) DM无眼底病变组 (NDR)血浆 VEGF、IFN- γ和 NO含量均较健康人明显增高 (P<0 .0 5 ;P<0 .0 5 ;P<0 .0 1)。 (2 )背景型 DR组 VEGF含量明显高于 NDR组而 IFN- γ含量明显低于 NDR组 (P<0 .0 5 ;P<0 .0 1)。 (3)增生性 DR组 VEGF、IFN- γ和 NO含量均较 BDR组明显降低 (P<0 .0 5 ;P<0 .0 1;P<0 .0 1) ,NDR组 VEGF与 NO含量呈直线正相关 (r=0 .30 ;P<0 .0 5 ) ,其它各组均无相关性。结论 VEGF过度表达及 IFN- γ、NO进行性减少在 DR新生血管的发生发展中起重要作用 ,其代谢紊乱是导致 DR发生中进展性微血管病变的重要因素。
Objective To investigate the effect of cell growth factors and nitric oxide on the pathogenesis of type 2 diabetes mellitus (DM).Methods Serum vascular endothelial growth factor (VEGF),γ interferon (IFN γ) and nitric oxide (NO) were determined by competi,tiveavidin biotin enzyme linked immunosorbent assay and spectrophotometer in 74 diabetes mellitus and 30 health controls.The relationship between these factors and glucosilated hemoglobin (GHbA1c)、 fasting blood glucose (FBG) and creatinine (Cr) were analysed.Results (1) Serum VEGF、IFN γ and NO in non retinop athy DMa group (NDR) increased significantly more than those in the control ( P <0.05; P <0.05; P <0.01 respectively).(2)VEGF in background DR group was significantly higher than that in NDR group ( P <0.05),while IFN γ in the former were significantly lower than those in the latter.( P <0.01; P <0.05).(3)VEGF IFN γ and NO in proliferative DR group were all lower than those in BDR( P <0.05; P <0.01; P <0.01).The positive linear correlation existed between VEGF and NO in NDR group,and no correlation existed amoug other groups.Conclusion Over expression of VEGF and the decrease of IFN γ,and NO played an important role in the occurence and development of DR neorescularigation.The metabolism disturbance of these factors was a vital factor in progressive microangiopathy in DR.
