摘要
目的：研究颈前路空心钉内固定术治疗创伤性不稳定的寰枢椎齿状突骨折的临床疗效。方法选择接受颈前路空心钉内固定术治疗创伤性不稳定的寰枢椎齿状突骨折的患者25例，术后颈围固定3个月，比较手术前后骨折复位、固定以及骨折愈合情况，考察头颈部临床症状是否消除以及术后并发症。结果所有25例患者均获随访，随访时间6～24个月，平均15个月。齿状突骨折全部愈合，无内植物松动、断裂，伤口感染及其它并发症。6例肌力减弱及肢体麻木的患者于术后3～4个月内恢复。结论目前治疗齿状突骨折的方法很多，但颈前路加压螺钉内固定治疗齿状突骨折具有操作简单，固定可靠，愈合率高等优点，是该类型骨折的治疗首选。
Objective To study the effect of anterior hollow screw fixation in the treatment of traumatic atlantoaxial instability odontoid fractures .Methods 25 patients with traumatic atlantoaxial instability odontoid frac-tures who received anterior hollow screw fixation were chosen .The neck was fixed for 3 months after operation .The fracture reduction ,fixation and fracture healing ,the elimination of the head and neck clinical symptoms and postopera-tive complications were examined and compared before and after surgery .Results All 25 patients were followed up for 6 to 24 months,an average of 15 months.Odontoid fractures healed without implant loosening ,fracture,wound in-fections and other complications .6 cases of muscle weakness and numbness recovered after 3 to 4 months.Conclusion The current treatment of odontoid fractures in many ways ,but the anterior compression screw fixation in the treatment of odontoid fractures has advantages of simple operation ,reliable fixation,high healing rate,etc.,it is the first choice for this type of fracture .
