摘要
目的：探讨强化心理护理干预对白血病患儿治疗依从性的影响。方法选择2009年8月-2012年8月收治的白血病患儿214例，按照随机数字表法随机分成观察组与对照组各107例，对照组给予常规护理，观察组在对照组护理的基础上进行强化心理护理干预。分别采用改良耶鲁围手术期焦虑量表（mYPAS）、社会支持量表（SSRS）评价两组患儿焦虑水平及社会支持程度，并比较两组治疗依从性。结果观察组患儿依从性良好率为87．85％，对照组为68．22％，差异有统计学意义（χ2＝12．024， P＜0．05）。观察组患儿社会支持总分、主观支持、客观支持、支持利用度得分分别为（41．14±6．34），（24．95±3．61），（10．52±3．33），（6．95±2．86）分，均高于对照组的（39．01±4．68），（21．74±5．29），（8．78±3．46），（5．89±2．53）分，两组比较差异有统计学意义（t分别为2．796，5．185，3．748，2．872；P＜0．05）。观察组mYPAS评分为（32．21±3．41）分，低于对照组的（35．89±3．36）分，差异有统计学意义（t＝-7．952，P＜0．05）。结论在常规护理的基础上联合强化心理护理干预，减轻了白血病患儿的焦虑程度，提高了白血病患儿的治疗依从性和社会支持水平，有助于患儿顺利完成化疗周期的治疗。
Objective To explore the effect of intensive psychological nursing intervention on treatment compliance of children with leukemia .Methods Two hundred and fourteen children with leukemia from August 2009 to August 2012 were divided into the control group and observation group according to the random number table, each with 107 cases.The control group received the routine nursing , and the observation group received the intensive psychological nursing intervention on the basis of routine nursing .The level of anxiety and the degree of social support were respectively evaluated by the modified Yale preoperative anxiety scale （ mYPAS） and social support rating scale （ SSRS ） , and the treatment compliance was compared between the two groups .＆amp;nbsp;Results The rate of compliance was 87.85%in the observation group, and was 68.22%in the control group, and the difference was statistically significant （χ2 =12.024, P〈0.05）.The scores of total social support, subjective support, objective support, the utilization of support were respectively （41.14 ±6.34）, （24.95 ± 3.61）, （10.52 ±3.33）, （6.95 ±2.86） in the observation group, and were higher than （39.01 ±4.68）, （21.74 ±5.29）, （8.78 ±3.46）, （5.89 ±2.53） in the control group, and the differences were statistically significant （t=2.796, 5.185, 3.748, 2.872, respectively;P〈0.05）.The score of mYPAS was （32.21 ± 3.41） in the observation group, and was lower than （35.89 ±3.36） in the control group, and the difference was statistically significant （t=-7.952, P〈0.05）.Conclusions Application of the intensive psychological nursing intervention on basis of routine nursing can reduce the degree of anxiety in children with leukemia , and improve the treatment compliance and the level of social support , and will help to successfully accomplish the cycle of chemotherapy treatment for children .
