摘要
目的 :为椎弓根螺钉内固定术提供解剖学资料。方法 :在 10 0例脊柱骨标本上测量螺钉通道长 ,并对其中 30例做CT断层摄影 ,观测螺钉通道的骨性结构。结果 :以下节段LS值相近 ,其平均值分别为 :C2 ~C7:32 5mm ,T1~T4 :36 6mm ,T5~T12 :43 3mm ,L1~S1:5 2 4mm。C2 ~S1侧别间均无显著性差异 (P >0 0 5 ) ,而性别间差异均为男大于女且差异有显著性 (C2 ~C5:P <0 0 5 )或极显著性 (C6~S1:P <0 0 1)。螺钉通道除关节突部有双层骨皮质外 ,其余均为骨松质。结论 :C2 ~S1的LS值为逐渐增大趋势 ;颈胸段椎弓根外缘骨皮质极薄 ,对螺钉几乎无阻力 ;螺钉的固定作用主要为关节突和椎弓根。
Objective:To supply the anatomical data for pedicle screw fixation.Methods:The length of screw channel have been measured on 100 intact skeletons and the bone structure of screw channel have been observed by means of CT laminagraphy on 30.Results:The average values of LS with follow regions were C 2- C 7:325mm,T1-T 4:366mm,T 5-T 12 :433mm,L 1-S 1:524mm.LS from C 2 to S 1 suggested that there were no significant difference between two sides (P>005) while difference did exist between sexes that from male being greater than that from female in C 2-C 5(P<005) and there were extremely significant differences in C 6-S 1(P<001).The spongy bone were exist mainly except articular process with two layer cortical bone on the screw channel.Conclusion:LS showed gradual increase downwards from C 2 to S 1.The thickness of pedicle lateral cortex were extremely thin and with no obstruction nearly to the screw at the cervical and thoracic regions.The fixed action of screw were articular process and pedicle mainly.
