摘要
目的探讨膝关节镜下关节清理术治疗保守治疗无效的膝关节骨性关节炎（KOA）的临床疗效。方法回顾性分析接受关节镜手术治疗的76例KOA患者的临床资料,手术时患者年龄为52-79岁,其中男25例,女51例;左膝38例,右膝42例,双膝4例。根据手术前后膝关节临床评分系统（KSS）观察关节疼痛和功能改善情况。结果术后1-2年随访,76例患者KSS临床评分平均92分,功能评分除1例外均可达90分,优良率高达100%;术后3-4年随访,36例患者KSS临床评分平均86分,功能评分平均81分,优良率为94.4%。结论关节镜下关节清理术治疗KOA患者应根据临床症状及术中所见清除关节内存在的机械性磨损因素,同时术后配合口服及关节内给药可以获得较满意的疗效。
Objective To explore the clinical effects of arthroscopic debridement in the treatment of knee osteoarthritis（KOA）.Methods The clinical materials of 76 patients with KOA including 25 male patients and 51 female patients who underwent arthroscopic debridement were retrospectively analyzed. The age range of the patients was from 52 to 79 years old. The osteoarthritis in left knees, right knees and both kneeswere 38 patients,42 patients and 4 patients respectively. The knee clinical scoring system（KSS）was used to evaluate the condition of knee pain and joint improvement before and after operation. Results The mean KSS of 76 patients was 92and the functional scores for all paitents were 90 except 1 patient. It revealed that and the recovery rate came up to 100% after 1 to 2 years of follow- up visit. Besides, the mean KSS of 36 patients was 86 and the mean functional score was 81. The recovery rate was 94.4% after 3 to 4 years of follow- up visits.Conclusion Clinical symptoms and intra- articular mechanical wearing factors combined with the postoperative oral and intra- articular administration of drugs should be considered when arthroscopic debridement was applied to treat KOA to obtain satisfactory effect.
