摘要
目的:探讨我院门诊中药药房处方差错的常见原因分析及控制措施。方法:对2011年1-12月门诊中药房调剂出错的处方进行回顾性分析,首先分析处方差错的原因,并针对各种因素采取相应的干预措施1年之后,再统计2012年1-12月门诊中药房调剂出错的处方及原因,对实施干预措施前后的中成药调剂差错、中草药调剂差错、患者满意度及患者的生存质量进行比较。结果:2011年度中成药调剂差错例数为32例,明显多于2012年度(10例)(P<0.01);2011年度中草药调剂差错例数为21例,明显多于2012年度(P<0.01);2011年度门诊患者对中药治疗满意度为74.0%,明显小于2012年度(96.0%)(P<0.05);根据生存质量评价标准,2011年度生存质量总得分为(102±5)分,明显小于2012年度(P<0.01)。结论:采取积极的、正确的中药房处方差错干预对策,能够明显改善医院门诊中药房的调剂质量、患者满意度及生存质量。
Objective: To investigate the hospital outpatient medicine pharmacy prescription errors common cause analysis and control measures. Methods: Outpatients prescription pharmacy dispensing errors were retrospectively analyzed from January to December in 2011. The reasons of prescription errors were analyzed firstly, and for a variety of factors to take appropriate intervention after 1 year, then count outpatient prescription pharmacy dispensing errors and reasons from January to December in 2012; The implementation of interventions before and after medicine dispensing errors, herbal spice errors, patient satisfac- tion and quality of life of patients were compared. Results: There were 32 cases of medicine dispensing errors in 2011 Annu- al, which was significantly more than that in 2012 (10 cases) (P〈0.01); There were 21 cases of herbal dispensing errors in 2011 annual, which was significantly more than that in r 2012 (7 cases) (P〈0.01); the outpatients right medicine treatment satisfaction was 74.0% in 2011 annual, which was significantly lower than that in 2012 (96.0%) (P〈0.05); based on the qual- ity of life evaluation criteria, the life quality score was (102±5) in 2011 annual, which was significantly less than that in 2012 annual with (133±8) (P〈0.01). Conclusion: A positive and correct errors in the pharmacy prescription intervention mea- sures can significantly improve the hospital outpatient pharmacy dispensing quality, patient satisfaction and quality of life.
