摘要
目的分析笔者所在医院门诊西药房处方调剂差错的原因,并采取相关防范措施,保障患者用药安全。方法采用回顾性分析,对2009~2011年门诊西药房发生的调剂差错进行统计分析。结果 2009~2011年医院门诊西药房共发生处方调剂差错110例,其中2009年42例,2010年36例,2011年32例,差错例数呈逐年下降。差错类型包括:品种差错50例,占45.5%;数量差错40例,占36.4%;错发患者差错12例,占10.9%;用法用量差错4例,占3.6%;其他差错4例,占3.6%。结论通过对医院门诊西药房处方调剂差错的分析,提出改进电子处方系统、规范管理、改善环境、品管圈活动等防范措施,提高门诊药房的调剂质量,保障患者用药安全。
Objective To analyze the reasons of prescription dispensing errors of western medicine in outpatient pharmacy and take some precautions to ensure the medication safety of patients. Methods Retrospective analysis of the prescription dispensing errors in our outpatient pharmacy from 2009 to 2011. Results There were 1 l0 cases of prescription dispensing errors in our outpatient pharmacy from 2009 to 2011,including 42 cases in 2009,36 cases in 2010 and 32 cases in 2011.The number of dispensing errors had decreased year after year. The number of errors and the percentage of errors in drug varieties,quantity,erroneous patient and usage^dosage form were 50,40,12,4,4 and 45.5%,36.4%,10.9%,3.6%,3.6% respectively. Conclusion Through the analysis of prescription dispensing errors in our outpatient pharmacy,the powerful pharmaceutical management and preventing measures according to the related errors have been taken,such as improving the electronic system of prescription,standardizing the management,improving the environment,promoting the quality control circle activities and so on,which can efficiently improve the quality of prescription dispensing in outpatient dispensary,and ensure the safety of drug use.
