摘要
目的探讨D-二聚体、CRP、PCT水平对老年社区获得性肺炎严重程度评估及预后的关系。方法选择128例老年社区获得性肺炎患者,将其资料进行收集,根据肺PSI、CURB-65评分以及重症肺炎判定标准进行分组,观察D-二聚体、CRP、PCT水平与不同分组之间的相关性。结果与低危组比较,中危组、高危组患者的D-二聚体、CRP、PCT水平均增长明显,差异具有统计学意义(P<0.05)。与Ⅰ组比较,Ⅱ组、Ⅲ组患者的D-二聚体、CRP、PCT水平均增长明显,差异具有统计学意义(P<0.05)。与重症肺炎组比较,非重症肺炎组的D-二聚体、CRP、PCT水平均降低明显,差异具有统计学意义(P<0.05)。与生存组比较,死亡组患者的D-二聚体、CRP、PCT水平均增长明显,差异具有统计学意义(P<0.05)。D-二聚体水平与CRP水平、预后均具有相关性(r=0.526,r=0.630;P<0.01)。PCT与预后之间有显著相关性(r=0.766,P<0.01)。结论在老年社区获得性肺炎患者中,随着病情的加重,其D-二聚体、CRP、PCT水平呈现不断升高的趋势,而D-二聚体及PCT水平可作为患者的预后指标应用于临床。
Objective To discuss the application of D-dipolymer/CRP /PCT levelsin the assessment of severe degree of elderly community acquired pneumoniaand it ＇s prognosis. Methods 128 cases of elderly community acquired pneumonia patients were employed in the study and collected their data. They were divided into different groups according to PSI / CURB-65 scores and severe pneumonia judge criteria. The relativity between D-dipolymer / CRP / PCT levelsand severe degree of pneumonia were observed. Results D-dipolymer / CRP / PCT levelsin middle-and high-risk group significantly higher than those in low-risk group（ P 0. 05）. D-dipolymer / CRP / PCT levelsin group II and group III significantly higher than those in group I（ P 0. 05）. Compared with severe pneumonia group, D-dipolymer / CRP / PCT levels in non severe pneumonia group obviously decreased（ P 0. 05）. Compared with survival group,D-dipolymer / CRP / PCT levels in death group increased significantly（ P 0. 05）. D-dipolymer level was correlated to CRP level and prognosis（ r = 0. 526,r = 0. 630; P 0. 01）. PCT had relativity with prognosis（ r = 0. 766,P 0. 01）. Conclusions D-dipolymer / CRP / PCT levels increased as elderly community acquired pneumonia got worse,and D-dipolymer / PCT levels could be used as the index of prognosis.
