摘要
目的：探讨保留乳头、乳晕乳腺癌术后背阔肌皮瓣加假体一期置入乳房再造的临床效果。方法选择2011年12月～2013年10月我院16例乳腺癌患者作为观察组，该组病例采取保留乳头、乳晕乳腺癌术后背阔肌皮瓣加假体一期置入乳房再造的方案治疗，选择同期收治的16例乳腺癌患者作对照，该组病例采取传统改良根治术治疗，术后未行乳房再造，随访比较两组的存活率、复发率、并发症发生率和生存质量，并对观察组的美容效果进行调查。结果两组患者均手术成功，术后均无严重手术并发症，随访至今观察组存活率为100%，未见复发和远处转移病例，观察组的复发率、存活率同对照组比较无显著性差异（P＞0.05）；观察组的生活质量评分为（77.2±4.3）分，此指标明显好于对照组（P＜0.05），观察组对本次美容效果的优良率为93.75%。结论保留乳头、乳晕乳腺癌术后背阔肌皮瓣加假体一期置入乳房再造的临床效果良好，安全性和美容效果为患者所肯定，值得临床借鉴推广。
Objective To investigate clinical efficacy of breast reconstruction with latissimus dorsi flap plus prosthesis implantation surgery after nipple-sparingmodified radicalmastectomy of breast cancer. Methods 16 patients with breast cancer were selected as observation group, who received treatment in our hospital from December 2011 to October 2013. Breast reconstruction with latissimus dorsi flap plus prosthesis implantation surgery after nipple-sparingmodified radicalmastectomy of breast cancer was taken to treat the cases. 16 patients with breast cancer treated in the same period was selected as control group, who taked traditional modified radical mastectomy without breast reconstruction. Survival, recurrence, morbidity and quality of life of both groups were compared by follow-up. And cosmetic effect of observation group was also investigated. Results The surgery of both groups were all successful,without serious complications. The survival rate of observation group was 100%, and there were no local recurrence and distant metastasis. Compared with the control group, recurrence and survival rate had no significant difference（P〉 0.05）. Quality of life scores of observation group was （77.2±4.3）points,and the indicator was significantly better than the control group（P〈0.05）.Cosmetic effect of observation group was 93.75%. Conclusion Breast reconstruction with latissimus dorsi flap plus prosthesis implantation surgery after nipple-sparingmodified radicalmastectomy of breast cancer has a good clinical efficacy. Patients affirm its safety and cosmetic effect, so it is worthy of promotion.
