摘要
目的 研究脑卒中急性期的抑郁症状及其影响因素。方法 对 2 0 6例脑卒中患者在发病 1月内使用Zung抑郁自评量表及自制问卷、神经功能损害量表 (DNF)、伴发疾病量表、简易智能速查表 (MMSE)、生活事件量表 (LES)、日常生活能力量表 (ADL)、社会支持量表进行评定。结果 2 0 6例患者中有 10 2例出现抑郁症状 ,其发生率为 49 5 %。单因素及多因素分析筛选出影响脑卒中后急性期抑郁症状的因素与下列 15项有关 :神经功能损伤程度、社会支持 (总分、主观分、客观分 )、生活事件总分、患者对治疗的依从性、家庭负担、病变部位、病前是否吸烟、生活能力总分、智能状况、本人对愈后的评价、家庭关系、职业、居住地。结论 脑卒中急性期抑郁症是急性脑卒中常见并发症、受多种因素影响 。
Objective To study depressive symptoms and effective factors of brain apoplectic patients in acute period.Methods 206 patients with brain apoplexy were assessed by Zung Self-Evaluating Scale of Depression,Self-made Questionnaire,Damage of Neurofunction Scales (DNF),Intercurrent Disease Scale,Mini-Mental State Examination (MMSE),Life Events Scale (LES),Activity of Daily Living Scale (ADL) and Social Support Scale within first month of brain apoplexy onset.Results Among 206 cases,102 showed depressive symptoms,whose rate of incidence was 49 5%.By using analysis of single factor and multifactor,15 factors were determined to correlate with depression,ie degree of neurofunction damage,score of social support(total,subjective and objective score),total score of living events,cooperation of patients in treating,family burden,site of disease,smoking history,total score of daily-living activity,state of intelligence,self-evaluation of prognosis,family relationship,occupation and living area.Conclusion Post-apoplectic depression is the common complication in patients with acute brain apoplexy,which is affected by many factors and more attention should be paid to its prevention and treatment.
