摘要
目的 对比原发性闭角型青光眼患者在不同眼压下手术的疗效.方法 选取2011年1月~2013年4月我院收治的原发性闭角型青光眼行小梁切除手术的患者160例（共184眼）,根据眼压不同分为观察组和对照组,每组80例,观察组为持续3 d治疗且眼压仍≥40 mmHg的患者,对照组为可用药物维持眼压＜40 mmHg的患者.住院时和术前检测两组视力和眼压,观察组在术前静脉滴注20%甘露醇250 ml,而对照组在术前未注射高渗剂.观察记录两组术中并发症,和术后1 d和3个月的视力、眼压和并发症.结果术后并发症及术后1 d和术后3个月眼压、视力,两组无统计学差异（P＞0.05）;两组术后3个月的并发症比较,观察组明显高于对照组（P＜0.05）.结论 不同眼压对原发性闭角型青光眼患者手术的疗效（视力、眼压和术中并发症）没有显著影响,但高眼压者术后并发症的发生率明显高于低眼压者.
Objective To make comparison of curative effects among patients with primary angle-closure glaucoma with different intraocular pressure who received the surgery treatment. Methods 160 patients( 184 eyes) with primary angle-closure glaucoma who received trabecular resection between January 2011 and April 2013 were selected,and were divided into observation and control groups with 80 cases in each group according to their different intraocular pressure. The patients in the observation group were those who had taken the treatment for consecutive 3 d and whose intraocular pressure was still ≥40 mmHg. The patients of the control group were those whose intraocular pressure could be kept < 40 mmHg with medicine. The visual acuity and intraocular pressure were detected at the time of hospitalization and before the operation. The observation group received the intravenous drip with 250 ml 20% mannitol,but the control group was not injected with any hypertonic agents. Recording was made in the two groups' complications during the operation and the visual acuity,intraocular pressure,and complications 1 d and 3 months after the operation. Results Between the two groups,there was no significant difference in the postoperative complications and the intraocular pressure and visual acuity 1 d and 3 months after the operation( P > 0. 05). The incidence of complications 3 months after the operation in the observation group was higher than that in the control group( P < 0. 05). Conclusion Different intraocular pressure has no obvious influence on the operation curative effects( visual acuity,intraocular pressure,and complications during the operation) of patients with primary angle-closure glaucoma. However,the incidence of postoperative complications in the patients with high intraocular pressure is significantly higher than that in those with low intraocular pressure.
