摘要
目的探讨抗骨质疏松药物在老年股骨粗隆间骨折动力髋螺钉(DHS)内固定术后治疗中的效果。方法将107例老年股骨粗隆间骨折随机分为治疗组和对照组。治疗组54例,DHS内固定术后第2天应用抗骨质疏松药物治疗;对照组53例,DHS内固定术后未用抗骨质疏松药物治疗。结果所有患者均获得12个月以上随访。治疗组骨折愈合时间平均(13.5±0.9)周,比对照组平均(15.5±1.5)周短;治疗组并发症发生率(11.1%)低于对照组(26.4%),髋关节功能Parker评分优良率(92.6%)高于对照组(77.4%),差异均有统计学意义(P<0.05)。治疗组抗骨质疏松治疗后健侧股骨颈、Ward三角、股骨大粗隆骨密度明显增高,差异有统计学意义(P<0.05)。结论抗骨质疏松药物(鲑鱼降钙素、阿法骨化醇、维生素D咀嚼钙片)应用对老年股骨粗隆间骨折DHS内固定术后有促进骨折愈合、降低手术并发症发生率和增加骨密度的作用。
Objective To evaluate the clinical efficacy of antiosteoporotic drugs for senile osteoporotie femoral intertrchanterie fracture after internal fixation with dynamic hip serew（DHS）. Methods All cases were randomly divided into two groups, the treatment group（54 eases） and the control group（53 eases）. The treatment group was applied Salmon Calcitonin, Alfacaleidol and calcium tablet after operation, while the control group was not. Results All patients were followed up more than 12 months. The fracture healing time of treatment group was （13.5±0.9）weeks, which was shorter than that of control group＇s （15.5±1.5）weeks. The incidence of surgical complications of treatment group was 11.1%, which was less than that of control group＇s 26.4%. The excellent and good rate of Parker hip score of treatment group was 92.6%, which was higher than that of control group＇s 77.4%. There＇re signitleant differences （P 〈0.05）. In treatment group, antiosteoporotic drugs were used post-operation. In femoral neck of healthy side, Ward triangle, the greater trochanter of the femur, the BMD （bone mineral density） was significantly inereased, there＇s significant difference （P 〈0.05）. Conclusion Salmon calcitonin, Alfaealcidol and Calcium tablet have the function of promoting the fracture healing, reducing the incidence of complications and significantly increasing BMD for senile osteoporotic femoral intertrochanteric fracture.
