血清前白蛋白、α-L-岩藻糖苷酶与2型糖尿病患者糖尿病肾病的关系研究 被引量:4

摘要 目的:探讨血清前白蛋白(PA)、α-L-岩藻糖苷酶(AFU)水平与2型糖尿病(T2DM)患者糖尿病肾病的关系。方法:选择T2DM患者120例,根据患者24 h尿mALB将其分为NDN组(45例)和DN组(75例)。检测所有研究对象的PA、AFU、空腹血糖(FPG)、糖化血红蛋白(HbA1c)及24 h尿mALB等。对数据进行相关性分析。结果:①DN组的AFU较NDN组明显升高,而PA明显降低,差异有统计学意义(P<0.01)。②两组AFU均与HbA1c呈正相关(r=0.288&0.665,P<0.05),PA均与HbA1c呈负相关(r=-0.439&-0.774,P<0.05)。③两组PA与AFU之间均呈负相关(r=-0.652&-0.833,P<0.05)。结论:糖尿病患者血中AFU、PA水平可反映患者糖代谢控制的情况。AFU水平增高,PA水平降低与糖尿病肾病的发生、发展可能存在某种联系。联合监测患者血AFU、PA、尿mALB,对早期诊断及控制糖尿病肾病有重要意义。 Objectire: To explore the correlation between serum prealbumin, α-L-fucosidase and diabetic nephropathy in patients with type 2 diabetes mellitus (T2DM). Methods: 120 cases of T2DM were divided into simple diabetic group (NDN group, 45 cases) and diabetic nephropathy group (DN group, 75 cases) according to the excretion rate of 24-hour urinary micro albumin (mALB). Serum prealbumin (PA) , α-L-fucosidase (AFU) , glucose (FPG) , glycosylated hemo- globin (HbAlc) and 24 hours urine micro albumin (mALB) were tested in all patients. The cor- relation was analyzed. Results: (1)DN group had a higher level of AFU and a lower level of PA than NDN group. There was significant difference (P 〈 0. 01 ). (2)There were positive correlations between AFU and HbAlc levels in both groups( r =0. 288 & 0. 665 ,P 〈0.05 ) , and negative correlations between AFU and HbA1 c levels( r = - 0. 439 & - 0. 774,P 〈 0.05 ). (3)There were negative correlations between AFU and PA in both groups( r= -0. 652 & -0. 833 ,P 〈 0. 05 ). Conclusion: The levels of AFU, PA in T2DM patients may reflect the control of blood sugar levels. There may be any correlations between the increased level of AFU, decreased level of PA and diabetic nephropathy. Combined detection of serum levels of AFU, PA, may be helpful for early diagnosis and control of diabetic nephropathy.
