内镜辅助微血管减压治疗原发性三叉神经痛及面肌痉挛 被引量:1

摘要 目的通过内镜辅助显微外科技术,找到卡压三叉及面神经的责任血管,解除神经长期卡压引起的脱髓鞘改变,从而消除症状。方法患者术前头颅MRI检查排除肿瘤、动脉瘤等,磁共振断层血管成像检查初步确定存在责任血管;术中显微镜下运用小脑和脑神经"零牵拉"技术,探查三叉神经及面神经出脑干根部,内镜辅助找到责任血管,用合适大小涤纶棉团垫开,并梳理神经根部血管及蛛网膜粘连达到充分减压,罂粟碱稀释后冲洗手术区,然后关颅。结果 12例患者手术均取得良好效果,无小脑迟发性出血、脑脊液漏、颅内感染、继发脑干缺血等并发症,3例三叉神经痛患者术后出现口周疱疹;2例面肌痉挛患者出现一过性轻度面瘫,听力下降。结论微血管减压是目前首选有效治疗原发性三叉神经痛及面肌痉挛的方法。
