摘要
目的总结显微手术治疗面肌痉挛、三叉神经痛和舌咽神经痛的经验,分析其疗效及并发症。方法微血管减压手术治疗各类颅神经疾病1 002例(三叉神经痛243例,面肌痉挛757例,舌咽神经痛2例)。术前核磁共振检查了解病因及相应颅神经周围是否存在责任血管及其来源与走向,根据Jannetta等所述方法,于全麻下侧卧位进行手术。结果三叉神经痛患者手术有效率为94.2%,面肌痉挛患者为92.3%,2例舌咽神经痛术后症状均消失。手术并发症发生率为9.7%,严重并发症4例,死亡率为0.3%,没有致残病例。平均随访39个月,复发率为5.2%。结论显微手术治疗颅神经疾病疗效确切,良好的显微外科技术是降低并发症的关键。微血管减压术是高风险手术,必须重视术后病情突变的风险。
Objective To investigate the efficacy and complications of microvascular decompression for hemifacial spasm, trigeminal neuralgia and glossopharyngeal neuralgia. Methods One thousand and two patients were treated with microvascular decompression from August 1994 to December 2005, of whom 243 were trigeminal neuralgia, 757 facial hemispasm and two glossopharyngeal neuralgia. Preoperative MRI was performed to gain an idea of the etiology and the condition of the offending vessels. Results The effective rates for patients with hemifacial spasm and tri- geminal neuralgia were 92.3% and 94.2% , respectively. Two patients with glossopharyngeal neuralgia were cured soon after operation. The incidence of overall complications was 9.7% with the mortality of 0.3%. Follow-up was made for an average of 39 months with a recurrence rate of 5.2%. Conclusion Microvascular decompression is an effective treatment for cranial rhizopathies. Mature surgical technique can greatly affect the outcome and incidence of complications. Careful postoperative management is well suggested to deal with the possible recurrence.
