摘要
目的探讨2型糖尿病(T2DM)患者左、右颈动脉内膜中层厚度(IMT)与心血管危险因素的关系。方法根据糖化血红蛋白1c(HbA1c)水平将272例T2DM患者分为控制满意组(HbA1c≤8%,A组,94例)和控制不满意组(HbA1c>8%,B组,178例),用超声多普勒检查双侧颈动脉内膜中层厚度,并测定血糖和血脂水平。结果 B组左侧IMT(L-IMT)和右侧IMT(R-IMT)均较A组增厚(P<0.05),空腹血糖、总胆固醇和低密度脂蛋白胆固醇也较A组增高(P<0.05)。A组L-IMT、R-IMT与年龄、收缩压、脉压呈正相关(P<0.05),与高密度脂蛋白胆固醇呈负相关(P<0.05)。B组L-IMT、R-IMT与年龄、脉压呈正相关(P<0.05)。结论良好的血糖控制,能够降低T2DM患者心血管疾病发生风险。
Objective To investigate the relationship between carotid artery intima-media thickness(IMT) and cardiovascular risk factors in patients with type 2 diabetes mellitus(T2DM). Methods According to glycosylated hemoglobin(HbAlc) level,272 T2DM patients were assigned to two groups of A(HbA1c≤8%, 94 cases) and B(HbAlc〉8%, 178 cases). Left and right IMT were examined by ultrasonography. Fasting blood glucose (FBG) and lipid levels were measured. Results Left-IMT (L-IMT), right-IMT (R-IMT), FBG, total cholesterol and low-density lipoprotein- cholesterol levels were higher in group B than those in group A (P^0. 05). There were positive correlations between L-IMT and age, systolic blood pressure, pulse pressure in group A. Negative correlations were found between high-density lipoprotein-cholesterol and L-IMT. Both L-IMT and R-IMT in group B were positively correlated to age and PP. Conclusion A better glycemic control in T2DM patients can reduce the risk for cardiovascular disease.
