摘要
目的探讨缺血性中风病急性期中医证候规律。方法选择30例缺血性中风病患者,分析其急性期和缓解期、急性期不同年龄段证候分布,以及急性期各证候组合情况。结果急性期风证25例、火热证23例、痰证22例、血瘀证14例、气虚证16例、阴虚阳亢证14例,缓解期分别为18、16、15、13、17、15例。急性期风证、火热证、痰证所占比例均高于缓解期,但仅风证具有统计学意义。急性期年龄≤60岁者火热证、痰证、血瘀证、阴虚阳亢证所占比例高于>60岁者(P均<0.05)。急性期同时存在4个证候16例,3个证候4例,2个证候8例,单一证候2例。其中,两证组合以风痰证、风火证、火痰证的组合为主,三证组合以风火阳亢证、风痰瘀阻证的组合为主,四证组合以风火痰瘀证、气虚血瘀风火证、气虚血瘀风痰证组合为主。结论缺血性中风病中医证候急性期以风证为主,风证、痰证、火热证等实证组合多见,年龄≤60岁者实证所占比例较高。
Objective To investigate the TCM syndrome regularity of acute phase of ischemic stroke.Methods Thirty patients with ischemic stroke were selected,and the syndrome distribution at different ages of acute and remission phases was analyzed as well as the syndromes of various combinations in the acute phase.Results There were 25 cases established diagnosis of wind syndrome,23 cases of hot syndrome,22 cases of phlegm syndrome,14 cases of blood stasis,16cases of qi deficiency and 14 cases of hyperactivity syndrome in acute patients,and there were respectively 18,16,15,13,17 and 15 cases in the remission phase.The wind syndrome,hot syndrome and phlegm syndrome in the acute phase were higher than the proportion in remission,but only the wind syndrome was statistically significant.The incidence of phlegm syndrome,blood stasis syndrome,hot syndrome and hyperactivity syndrome in patients aged 60 years was significantly higher than that in patients over 60 years in the acute ischemic stroke syndromes （all P ≤ 0.05）.In the acute phase,16 cases had four syndromes,4 cases had three syndromes,8 cases had two syndromes,and 2 cases had the sole syndrome.The two syndromes combinations mainly included the wind-phlegm,wind-fire and fire-phlegm,the three syndromes combinations mainly included the wind-fire-hyperactivity,wind-phlegm-blood stasis,and the four syndromes combinations mainly included the wind-fire-phlegm-blood stasis,qi deficiency-blood stasis-wind-phlegm and qi deficiency-blood stasis-wind-hot.Conclusions The syndrome distribution of ischemic stroke in the acute phase is mainly the wind syndrome.The empirical combinations of wind,phlegm and hot syndromes are more common,which have higher proportion in age ≤ 60 years.
